Brief Answer:
May be due to compression of cervical nerve root.

Detailed Answer:
Hello,

I have studied your case.
Due to compression of cervical nerve root there can be pain your neck and arm.
I will advise you to MRI cervical spine for better diagnosis.

For these symptoms analgesic and neurotropic medication can be started.
Till time, avoid lifting weights, Sit with support to back. You can consult physiotherapist for help.
Physiotherapy like ultrasound and interferential therapy will give quick relief.
I will advise to check your vit B12 and vit D3 level.
Another possibility of shoulder pericapsulitis can be associated with it.

You may consult physiotherapist for rehabilitation
He may start TENS ,or ultrasound which is helpful in your case.
You may need Shoulder manipulation under anaesthesia or arthroscopic adhesion release if no relief with physio.
As pain is on left side and you have old history of cardiac disease get your regular cardiac check up done.
