Brief Answer:
get a needle biopsy

Detailed Answer:
Hi,

Welcome to HealthcareMagic.

Painful lymph nodes are usually infective in origin. And 4mm lymph node is not a significant enlargement.

May be your on and off cold has spread to the sender inner ear and has caused the node to enlarge.

But as you have already taken course of antibiotics and antiinflammatory medicines and it has not regressed completely, so your doctor would have referred you to a specialist. So show once to an ENT specialist for a complete clinical evaluation to look into the cause of your on and off cold and the lymph node enlargement.

Also I would advice you to get a needle biopsy of that node if it doesn't regresses even after following the prescription of the specialist to help reach a diagnosis.

Hope i have been helpful.
