What Causes Nausea, Diarrhea And Abdominal Pain After Treating Severe Pancreatitis?

Question: Hello, I am a 60 y/o male with diabetes taking fast and long lasting insulin since 2006. Have been having pancreatitis since 2007, and in 2014 hospitalized with sever abdominal pain and given morphine drip and antibiotics x3 days. CT scan revealed severe pancreatic atrophy. EUS done in XXXXXXX 2016 revealed hyperechoic strands and hyperechoic foci. Report said no mass was found, however one photo had a notation of a mass, have been on enzymes, 400 mg per meal, and for first 2 weeks was feeling good when eating, no nausea or runny fatty stools, and blood sugars in normal range. Last 5 days experiencing nausea, runny fatty stools, abdominal pain mostly on right side, and blood sugars elevated (200 in am). Pain is a weekly situation and treated with 5/325 hydrocodone. Weight has fluctuated between 194 to 178. Is this a situation needing surgical treatment?

I am so sorry to hear about these symptoms that you have reported. I have also reviewed the attached images. There is no ned for surgical treatment here as there is no significant mass or dead tissue collection which requires excision. However, diarrhea after severe pancreatitis is very likely as there are insufficient enzymes to properly digest the food. I will suggest that your doctor reviews the dosages and adjust accordingly. Also, with no added insulin secretion because of a completely damaged pancreas, you may need more insulin than before for good blood glucose management.
