Posted on Wed, 19 Apr 2017
Question: Hi doc, last thursday March 09,2017 i visited obgyn, she put 2 tablets of misoprostol inside my vagina and that time she advised me also to take 2 tablets orally and she gave me 6 more tablets that i will take every 3 hours , after that i bleed but light only. On the other day she did the scan and said there's a separation happened already, so she repeat the same medication, she put 2 tablets inside my vagina and i took 2 tablets orally, and the remaining i take every 3 hours, i bleed but not so heavy, she scanned me again on the next day and said almost done, so she put 2 tablets again inside and i take 2 orally and gave more 6 tablets that i take every 3 hours but that time no more bleeding we repeat the medication until yesterday March 14.By the way im 6 weeks pregnant when i started the medication. My Question doc is the chance that it will become successful. Is there's a possibility that my pregnancy will still continue if i stop taking medicine? Thanks in advance

Hello
Thanks for trusting us with your health concern.
Please elaborate upon your question ?
Do you wish to continue the pregnancy, or do you wish to have an abortion ?
The dosage taken by you is incorrect, and not the right one to have an abortion.
Please clarify, so that I can assist you further.
Looking forward to hearing from you.
Take care.

As below

Detailed Answer:
Okay.
Let me outline the correct dosage for you then, and the correct procedure.
First and foremost, medical abortion should NOT be attempted without medical supervision.
1. Even if the home pregnancy test is positive, the pregnancy needs to be confirmed by a gynecologist and exact dating needs to be done, since these pills work only up to specific gestational age, and not beyond that. So do not act just after taking a pregnancy test at home.
2. You need to rule out possibility of ectopic pregnancy (pregnancy outside the uterus) which will also be done by a specialist, as taking medical pills in that scenario is disastrous and life threatening.
3. Medical abortion has also got chances of severe life threatening bleeding at times, for which you need to be under medical care.
4. It has a 5 % failure rate, following which you require surgical termination, which might be needed in an emergency also.
5. Unsupervised medical abortion might cause severe infection, retained products which maybe undetected. Hence it is important to have a follow up visit with your gynecologist two weeks AFTER taking the pills, at which it would be confirmed that the pregnancy was successfully and completely terminated.

That said and done, if this procedure is supervised by a doctor, it is quite safe and fairly successful.
I would give you a few guidelines about medical termination: Each pack contains 5 tablets -
1 tablet of Mifepristone…200 mg
4 tablets of Misoprostol, each containing Misoprostol…200 mcg

Mifipristone tablet is taken orally and 48 hours later, 2 tablets of Misoprostol are taken orally or vaginally.
Bleeding might begin at any time after taking Mifepristone or Misoprostol, but generally starts within 6 - 8 hours of taking Misoprostol tablets.
Another 2 tablets of Misoprostol are taken 24 hours later, irrespective of whether the bleeding has started or not.
If you vomit within 4 hours of using any pills, the dose has to be repeated..

For women at 49–63 days of gestation, if abortion has not occurred 4 hours after administration of misoprostol, a second dose of misoprostol 400 mcg may be administered vaginally or orally (depending upon preference and amount of bleeding).

The patient should return for a follow-up visit approximately 14 days after the administration of mifepristone. This visit is very important to confirm by clinical examination or ultrasonographic scan that a complete termination of pregnancy has occurred.

Patients who have an ongoing pregnancy at this visit have a risk of foetal malformation resulting from the treatment. Surgical termination is recommended to manage medical abortion treatment failures

As you can see, your doctor has given you a different dosage.
Now, just wait and watch for a week, and if there is no bleeding , then get a repeat ultrasound scan done, and if there is still evidence of pregnancy, surgical termination should be done.
Take care.

Yeah she gave me different dosage maybe because in this country no access for mifepristone only the Misoprostol.

Everytime i visit her, she always did the ultrasound scan and always mentioned to me that there's already separation of cell/zygote/sex happened. I'm confuse doc why no bleeding happened only on the 1st and 2nd day but light only.the rest of days of my medication from March 11-14 no blood come out.

Okay.
Probably that is the reason for the different dosage.
Sometimes bleeding may happen a few days late.
That is why I advised you to wait for one more week.
Most probably bleeding will happen during that time.
Take care.
