Brief Answer:
I recommend to discuss with your doctor the possible options.

Detailed Answer:
Hello! Thank you for asking on HCM!

I understand your concern and would advice you to discuss with your doctor, to see the possibility of adding other anti-hypertensives to your current therapy. As you have a pacemaker implantation, I would recommend with a great safety a beta-blocker like carvedilol; the dose must be up-titrated gradually till the maximal tolerated dose. A thiazide diuretics like hydrochlorothiazide or chlorthalidone would be of help to lower blood pressure. An alternative would be a loop diuretics, like torasemide. Other potential useful drugs are centrally acting drugs like Physiotens (monoxidine), or any alfa-blockers like Doxazosin, Prazosin, etc. So the anti-hypertensive armamentariun is very large, and we have the opportunity to choose the most appropriate combinations that work for a certain patient.

I would strongly advice you to discuss with your attending physician all the above options, and facing your specific medical profile, to choose the right representatives for you.
Hope to have been helpful. Greetings! Dr. Iliri
