Question: right side of head is tender to the touch around ear, eye back of head, have had sinus problem in the past could this be sinus

I can understand your concern. Such type of tenderness could be due to

: Sinusitis - It can be due to sinusitis like maxillary and frontal. These sinuses are present around eyes. Infection of our sinuses present as headache on one side, around eyes/sometimes ear till back of head, increases on forward bending/cold environment/allergy, heaviness of head, associated with postnasal discharge/nasal blockade. This headache is used to increase till evening. If your symptoms are matching, it could be sinusitis.

Consult ENT specialist for it. He will advise X ray or CT of paranasal air sinuses. After confirmation, treatment is given like
; steam inhalation 4-6 times daily
; avoid cold food/drinks, allergic factors
; eat hot/warm food/drinks, warm environment
; Tab Cetrizine-Paracetamol-Xylometazoline combination to relieve sinus wall edema/allergy and drain sinus secretions.
; Tab Coamoxyclav therapy (antibiotics) for 5-7 days.
Surgery is rarely needed in case of chronic sinusitis, resistant to treatment.

: Anpther possibility could be Migraine - It can be suspected if one sided headache, vomiting, blurred vision, redness of eyes, aura symptoms before pain like bad smell/bad taste, precipitated by stress/food, headache used to occur in particular season.

If your features are such, consult neurologist. Migraine is suspected only on symptoms. Treatment includes
: Preventive therapy like
Avoiding precipitating factors e.g. cheese/chocolate, mental stress, bright light, exertion
Few tablets which delay attack of Migraine. Any one of them can be taken. Drugs are Propranolol, Amitriptyline, Gabapentine, Pregabaline. The doctor will prescribe any one of them.

: Abortive therapy like
Painkillers which abort an attack of Migraine e.g. NSAIDs like Ibuprofen/Diclofenac/Aceclofenac are effective in initial stages, then Erogtamine or Triptans like Sumatriptan can be started if no response from NSAIDs.
Opioids like Morphine/Oxycodone can be taken if severe pain and no response.
