Question: Widal test of my wife(age 24) came as Typhi "O":POSITIVE 1:160 DILUTION and Typhi "H":POSITIVE 1:80 DILUTION , However para typhi"ah" and para typhi "bh" are negative. She is not showing any sign of fever but she is continually having headache, leg pain, body pain etc. She also has vitamin d deficiency as per test and little sign of thyroid...however doctor said she is in growing age so cant be having these issues because of thyroid.please suggest.

Brief Answer:
tests suggested

Detailed Answer:
Hi
I have read your query
the fact remains that the symptoms you have mentioned can be due to a large number of causes
Anemia, vitamin D deficiency, thyroid disorders and even typhoid.

fever is not an essential feature of typhoid. if she has had typhoid fever before, I would not give much thought to the Widal test. I would suggest Typhi DoT IgM/IgG. If this test is positive, we will have to treat for typhoid, else we can think of other causes. Testing thyroids will also be a good idea (done fasting).

you should start vitamin D replacement for her. I would like to see the actual report before I suggest the treatment for the same.

Please let me know the reports so i can help you further

Brief Answer:
Needs medicine

Detailed Answer:
On the basis of these reports, I would suggest starting Vitamin D supplementation 60000 Units weekly for 6 weeks and then retest it 6 weeks later.

Thyroid replacement should also start. A mild dose like 25 mcg would do. then we repeat the test again after 6 weeks.

I will repeat- test Typhi Dot IgM, and hb

best of luck
