What Causes Frequent Urination With Larva Eggs Attached To Anus?

Question: We travel as a family to St XXXXXXX every year and have done since 2008. In 2011, we visited the Island in both February and December. My two children are feral explorers and love catching local wildlife. In March 2012, within a week or two of returning from St XXXXXXX my now 10 year old son and daughter had itching to their feet. My son's foot first showed worm like burrows visible underneath the skin and he had terrible itching. I took him to the GP and he diagnosed cutaneous larva migrans. Some week later my daughter also had the worm like burrow appear on her feet. My son visited the GP and he was treated with a freeze to the area and then we had an after cream to apply. My daughter did not visit the GP, I telephoned my GP and having explained her symptoms were the same as my son, we agreed it was most likely cutaneous migrans. The GP kindly left a prescription for me to treat her myself so she did not have any freezing to the area. I very much regret not having taken her to the GP. Also in March 2012 we first treated XXXXXXX for thread-worm as she was getting up to pass urine several times during the night and this is when I first noticed small ivory eggs attached to her anus, pin head size. I could see no obvious threadworms in her stools however.

Almost 18 months on, my daughter continues to experience the following symptoms which appear to have a cycle: nausea, heartburn, bad breath, middle stomach ache, foul trumpeting wind from back passage, waking to pass urine several times during the early hours and she has dark circles under her eyes. I have spotted these eggs around her anus when her symptoms are at their worst on at least 3 occasions during this time. Every time I treat her (and family) with Ovec, I also treat my daughter with the second Ovec pill two weeks following. I notice a difference straight away following the second pill (two weeks later), she no longer wakes in the night to pass urine and she has a better appetite and does not complain so much of feeling nausea. Life returns to normal for her and she looks well and has no dark circles under her eyes. She is well for approximately two months then her symptoms appear again. More recently, since January she has told me she brings up little vomits at school. I have now asked her to try and spit it into a sample pot to show mummy.

I have been back to the GP several times. In October 2012 we suspected XXXXXXX had norovirus, she vomited for 10 days once a day and it was mostly phlegm with no food content, red circles under eyes and lower face red rash, no diarrhoea just raisin like stools very dark, high temperature of 38 plus degrees. This ended with the symptoms that appear regularly today, trumpeting foul flatulence, nausea, nighttime waking to pee (sometimes nothing comes out), dark circles under eyes and decreased appetite apart from anything sweet like chocolate which she devours. We had stool samples taken in April 2012 which do not show evidence of parasites/ova, though I did not provide her whole stool as it would not go in the pot. We also had full blood tests taken in 2012 and more recently in XXXXXXX 2014 ruling out h-pylori and inflammation and I believe coeliac disease.

My GP feels there is nothing further he can help with and so suggested he put me in touch with a Consultant Paediatrician. I took XXXXXXX along to a female Paediatrician. on Friday 16th May 2014. I provided her with a stool sample of that day and urine sample. We left with an open appointment.

May I ask you, reading my account, does it sound like XXXXXXX may have systemic problems connected to intestinal parasites picked up from St XXXXXXX back in 2011?

Brief Answer:
no connection,,, gastritis.

Detailed Answer:
Hi thanks for writing to us.

Looking at the query, you look very much worried.
First of all I would like you to bring down your worries as I don't feel the symptoms you have mentioned is any way connected to cutaneous larva migrans .

Systemic symptoms of cutaneous larva migrans include most probably cellulitis i.e localised tissue infection, that is the local part of the skin involved.

I feel that she might have chronic gastritis - either the stomach or or the duodenum part involved, which if confirmed by the relative tests, is easy to treat.

If the blood tests reveal a low hemoglobin count and suggests allergens in the blood, we ll have to rule out malabsorption syndromes or any reason for chronic blood loss in the stools if the tests are positive.

Chronic blood loss in micro quantities may cause frequent infections, fatigue, weight loss, dark circles under the eyes etc.

Meanwhile I would like you to avoid few food products which may cause these symptoms- wheat, artificial food coloring or sweetening agents, sodas , chocolates, jellys , non vegeterian foods (excluding egg).
Go low on protein loads ion the food.

I ll require the details of the test report that your child has undergone, so that I would be able to guide you properly.

Without first hand examination it would be difficult for me to comment further.

Kindly review with reports, wishing good health and speedy recovery, regards.
Do follow up.
