What Causes Falling GFR With History Of High BP?

Question: I have a gfr of 30 and it has been as low as 19. I understand creat and BUN levels in blood but I am not sure what a normal urine creatinine is. I also have a urine total protein of 24. I do have an appt. with my nephrologist tomorrow and wonder what I may expect from that appt.

I am so sorry to hear about this worsening kidney function. Do you have an idea what the cause of the chronic kidney disease is? Is the cause being well controlled to prevent further aggravation?

Urine creatinine should be approximately 15-25mg/kg over 24 hours. This means that for a person who weights 50kg, the highest value in 24hours should be 1250mg.

Normal urine protein should be less than 30g. What type of protein are you talking of here and what were the units of measurement. The highest acceptable creatinine/protein ration In urine is 200mg/g.

With the falling GFR, your nephrologist would be probing to identify the reasons for this and address them. Also, she may want to open up a discussion with you on what the way forward is as regards further management options should the GFR continue to fall to even lower values.

my kidney problems are because of long term high untreated BP which is now under control with use of Lisinopril. All I know about my urine creat and protein is that is was on a one time basic urine sample. (not a 24 urine collection). In previous appt with U of Utah Hospital nephrologist he only double-checked my kidney function and said come back in a year. That was about 2 months ago. in the past 2 weeks is when I found out about worsening function and so I am more concerned about it than I normally would be and I am really hoping there will be a plan of action besides just waiting for further developments. Thats why I hope you can give me some possible treatments I could expect. I do understand your advice is only your opinion. The more I learn about this, the more empowered and less anxious I will feel. Thank-you for your help.

As the GFR falls to values like 15 and below, options of hemodialysis would be discussed. Because hemodialysis is not a permanent solution, kidney transplant would also be considered as a long term and permanent solution.

In all, the chances as GFR deteriorates would be hemodialysis and kidney transplant in addition to preventing deterioration by appropriately managing the cause and aggravation factors.
