What Causes Extreme Fatigue And Nausea?

Question: These are my symptoms.
--extreme fatigue
--listless
--falling asleep at inappropriate times
--feeling very out of control when the sleepy feeling comes on
--feel very weak
--legs feel extremely week, especially when I try to get up
--feeling very shaky
--sometimes I have a shooting pain in my brain
--dull headache all of the time
--stiff neck
--once in a while, a wave of nausea will hit me.
I need help. I feel terrible and frightened.

If you would have presented to me as an outpatient,
I would have told you about the following etiologies which can cause these symptoms-
Anxiety
Thyroid disorder
Hypertension
Alcohol/Addictions
Chronic fatigue syndrome/Fibromyalgia
Psychosomatic

I would suggest you to get your blood pressure monitored,get thyroid function tests done and follow-up with results.

Please tell the following details-
1)Do you take alcohol?Any other addiction?
2)Do you have frequent anxiety or depression?

Please follow up with details.Meanwhile,start doing mental relaxation exercises/Yoga/Pranayama.

Dr, my blood pressure monitored on A regular basis and is under control with Metoprolol. I am not anxious, nor do I get depressed.I don't have enough alcohol to cause this type of problem. I might have two or three glasses a weekwith dinner. However, a friend has been telling me to get my thyroid checked.it does run in my family. What type of test needs to be done for that?
I had been seeing a neurologist for the last 15 years, and she left for practice so I'm late leaving no replacement.
My physical problems are:
-hypertension(controlled).............metoprolol
-restless leg syndrome,severe.......mirapex
-fibromyalgia..............................Low dose gabapentin. Elavil as needed
-severe neuropathy"....................nothing.

Is there a name for what is happening to me?

Hi XXXX,
Thanks for being in follow-up.

So,you have a strong background history with good enough reasons which can explain all your symptoms.However,one etiology that is most responsible is Fibromyalgia and what you are having can be very well said as 'Fibromyalgia syndrome'.
I have the following suggestions for you to land up in a better state:
1)Thyroid function tests-free T3,free T4 and TSH to be done.
2)Healthy balanced diet in terms of essential minerals and vitamins.
3)Antioxidants tablets to be taken on a regular basis.
4)Sleep hygiene
5)Regular exercises-brisk walk
6)Massage therapy
7)Adequate mental and physical relaxation-Don't overstress your lifestyle
8)2-3 times a week alcohol intake is a bit more in frequency and it should be limited
9)Don't have coffee or tea in the evening hours or at night time
10)Limit fluids intake in the evening hours,so as to avoid frequent urinary urge during sleep hours.

Regarding medicine therapy,you need to be put on clonazepam and amitriptyline.These two drugs should bring a substantial improvement in your symptoms,in addition to your existing drug therapy.PLEASE CONSULT YOUR PHYSICIAN TO START THESE MEDICINES.

A psychiatrist's opinion regarding the lifestyle changes to manage Fibromyalgia will be crucial.

Please follow up with Thyroid function results.Keep monitoring your Blood pressure.

Thank you very much for your answers and suggestions. Some of them I am already doing.
- I will certainly have my thyroid checked.
- never drink coffee or tea.
- I try not to drink any liquids before bedtime.
- I will certainly get back on my healthy diet that I was on.
- I have never taken antioxidant tablets, but I will start ASAP.
- I will also start getting massage therapy. Do I need a prescription for these?
- Excersise is in my furture.
- What exactly do you mean by "sleep hygiene"?
- I will cut back on the alcohol intake. I just wanted to tell you it is one glass
with dinner 2 or 3 times per week. Is that too much?
- I don't feel like I overstress my lifestyle, but maybe I do.
- Why would I need a Physchiatrist's opinion? I am a very mentally healthy,
happy, confident, well adjusted person. The last time I was depressed was 44
ago after my daughter was born. What am I missing here that I don't
understand?
- Regarding medications, she had me on amitriplyine, but I couln't function the
next day. I will try it again.

Thanks for being in follow-up.
I am glad you appreciated the response.Please find answers to your questions as under:
1)No prescription needed for massage therapy.Just find a good massage therapist and discuss the issues with him.He will do the needful.
2)Sleep hygiene refer to practices which are undertaken to ensure a proper night sleep and good daytime alertness.These are-
Avoid daytime naps
Avoiding brain stimulants like coffee,tea,alcohol during night time close to sleep
Exercise during the daytime but not during night just before sleep time.
Avoid food just before sleep
Relaxing bedtime routine-No disruptive conversations with family members at this time.
Alcohol at the dinner time 2-3 times a day,as I said above,stimulates the brain and disrupts sleep.Try to avoid it.
A psychiatrist can explain in an excellent way about the things or behaviour modifications I mentioned above as he is the specialist meant to do so.I am not referring you to one for any mental problems.
Start amitriptyline with low doses.You will adjust to it in 2-3 days.
