Brief Answer:
Possible sleep apnea

Detailed Answer:
Hello,

Thank you for using Healthcaremagic to post your question.
I read it carefully and I understand your concern.

It is difficult to tell for sure what your friend has because to determine a diagnosis will be needed full medical history, a physical exam and lots of labs. Your friend seem to have a lot of problems.

The most possible cause of that symptom you are describing is sleep apnea, expecially if your friend is obese and also if he has high blood pressure too. Sleep apnea is the stopping of the breathing while sleeping for more then 30 seconds, which leads to bad sleep during night and as a result these patients fall asleep during the day even while talking with other people.

From what you are telling me I would say he can have the congestive heart, because the problems he has lead to that. Congestive heart failure does not give that symptom in particular though, but more frequently gives shortness of breath at rest, swelling of the legs, general fatigue, ect.

Hope this is helpful and answers your question.
Let me know if you need more clarifications.
