What Causes Elevated SGOT And SGPT Levels After Taking TB Medication?

Question: My name is XXXXXXX and started taking TB medicine on 29th March after diagnosis. MY SGPT N SGOT was normal after 15 days of starting drugs but after test today the level is 4 times in both SGPT and SGOT . I took only beer from 22nd to 26th as was out on a vacation and took 8 beers in 4 days . Please help is that normal ??

I'm Dr. Shuba Hariprasad and will be answering your query today.

Alcohol is contraindicated while on medicines.

Drugs for TB(tuberculosis) are mostly metabolized in the liver. This is an added strain for the liver and may cause elevated enzymes themselves.
Since your initial levels (15days after surgery) was normal,this is unlikely

Alcohol is metabolized in the liver as well and puts an added strain on your liver and damages the cells, hence elevating liver enzymes.
I would ask you to avoid alcohol at any cost and put you on liver protective drugs like LIV52, N-acetyl cysteine.

I would also like to rule out pancreatic and gall bladder involvement (especially if you have any abdominal pain, fever, vomiting)
I recommend you see your physician right away.

Please do not take any medications without consulting your doctor first

Hello Doctor,

I think let me give u more details to answer my question completely to my satisfaction.
I started ATT on 29th March with Rcmex, Pyzina, Mycobutol , feburic and pyredxin ( night) . My LFT was SGOT and SGPT along with ALP was in normal range on a test being done on 6th April . I continued on medicine regularly and went on a small trip for 4 days ( 21st to 25th April) where had 6 bottle of very light beer NO HARD LIQUOR , NO SMOKING .

On 30th April again LFT test done where in SGOT was 167 ( 5-34 ) and SGPT was 214 ( 5-55 ) which came as alarming . I am a vegetarian and not taking any fried food .

Today again test done with another lab and SGOT was 184 ( 17-59 , normal range shown) N SGPT 225 ( 21-72 ) and Gamma GT is also down from 217 to 192 .

Now looking at the above situation i want to ask having SGPT an SGOT little higher is worry some though in two days test on 30th and 2nd the trend is reversing . I am not taking any beer any more and taking diet like rice , idli and khicdi to give rest to liver . Please let me know your detailed opinion after reading the above . Hope you will satisfy my question as I am little worried .

Thank you for the follow up.

I have read through your query carefully.

Beer contains alcohol in varying percentages - between 1-45%(unless it's non alcoholic).

6 bottles of very light beer may have been tolerated well, by the liver, if you were not on medications. This depends on your how much you drank and how fast and so on your Hydration status
It also depends on person to person.

Having it along with anti tubercular medications can put an added strain on your liver, as I mentioned in my earlier reply. This can be difficult for the liver to have and result in elevated enzyme levels (SGPT, SGOT, GGT).
Your lab results today show an increased (slight) in SGOT & Sgpt and slight reduction in GGT.

This may be due to variations in labs. Ideally repeating the tests in the same lab (a good one) is advised.

Your values don't seem to have changed much in two days, which is good news, as it rules out ongoing liver cell damage (which would lead to drastic increase in values even if you did it in a different lab).
Please note that anti tubercular medications do put a strain on the liver. This, alone, can cause raised enzyme levels which usually settles once the body has adjusted to the medications.
Occasionally, a change in treatment regime may be necessary if the liver cannot handle it.

I appreciate that you have chosen not to consume anymore beer/alcohol.

If you have any abdominal pain, unusual fatigue, muscle pain, Twitching, unexplained itching, yellow discoloration of eyes, palms, dark yellow urine, please see your doctor right away.

I understand how it must have been alarming to see raised levels after normal levels. However, you decided to check your blood and found out the elevated levels. Else it would have gone unnoticed.

Please note there is nothing to worry about. Just be cautious as your body and especially liver is going through a tough time now.
Even very light beer or a fever/headache pill like paracetamol can tip the scale and cause damage.
