Brief Answer: Kindly send details and pictures of area Detailed Answer: Hi, Welcome and thanks for posting your query to the forum. I can understand your concern for the dry skin/ rash and itching on ankle for last three days. There can be multiple possibilities of the rash and it requires close physical examination to rule out exact condition. I request you to kindly elaborate certain important points: - Do you feel any redness on the dry skin. - Are there any eruptions on the affected area. - What is the severity of itching- mild/ moderate/ severe. - Can you tell me the name of cream and ointment at home for psoriasis or at least the contents in it. It would be best if you can upload few good resolution pictures of the affected area. For that you have a feature to upload images by yourself at the right side of the query page. Or else, you can send as an attachment to YYYY@YYYY with the subject as "Attn: Dr. Sanjay Kumar Kanodia. Hope to hear soon from your side. Wish you good health. Regards. It is hard for me to tell if it is dry skin or not. I have been keeping it very moisturized. It appears as if the area on the left side is inflamed. The lines/bumps that you see are raised on the skin. This can be felt when rubbing your hand overtop of it. It's very irrupted and has a tingle to it, I think due to it being inflamed. There are no eruptions. It itches quite a bit and I very badly want to itch it, but have refrained. It is uncomfortable. It's only the left side that itches right now, It started after I accidentally itched it yesterday! The steroid creams that I have at home for my psoriasis are Talconex and Cyoderm (I think) I have an appointment with my dermatologist tomorrow at 1:30pm. If there is anything I can do to get some relief in the meantime, that would be great. Unless you think this is urgent/harmful and I should go to urgent care? Thank You. I attached more pics as requested. XXXXXXX

Brief Answer: Talconex cream & loratadine for two times Detailed Answer: Hello, Thanks for your follow up and sending important details along with pictures. The pictures shows really red looking areas with raised surface of skin. Though it is quite early to give you exact possible diagnosis but I suggest you certain important points till you meet your dermatologist: - Do cold compression or warm water compresses according to the weather condition. -Don't try to scratch or rub the area as can lead to further aggravation of the problem. - Keep your self away from hot and humid climate. - Wear cotton clothes and do not wear any synthetic or tight garments or stockings in touch with the affected skin. -If I were your treating dermatologist then would have suggested to apply a low potency steroid cream containing fluticasone or cortizone cream. For the time being period you can use Talconex cream as contains betamethasone dipropionate which will reduce the redness. -For itching I would have preferred to give an oral anti histamine like loratadine (claritin) two times a day if there is moderate to severe itching is present. The rash most likely appears to be due to accutane, so I suggest you to discuss the same point with your dermatologist. Therefore if dose adjustment is required then can be made accordingly as per your dermatologist's clinical examination. I hope these information's will help you. If you have further queries I will be happy to help. "Wish you good health and early recovery from the problem" Regards, Dr Sanjay Kumar Kanodia MD- Dermatology
