default
Posted on Fri, 2 Jan 2015
Question: I am having trouble urinating. I have to use a lot of pressure to get started and to continue, the same amount of pressure used to have a bowel movement is the amount of pressure i have to use. Then i don't feel like i have completely emptied my bladder. I also have left side flank pain.

Brief Answer:
Probably an obstruction to the bladder evacuation

Detailed Answer:
Hello,
Welcome to HCM and thanks for your query.
You have an obstruction to the urine outflow after the bladder. It can be from an obstruction at the bladder neck or in the urethra. It can also result from a neurological problem interfering with the bladder contraction.
If you have a micturating cystourethrogram, it will make things clearer

How is this treated and diagnosed?

Brief Answer:
Micturating cystourethrogram

Detailed Answer:
It depends on the result of the micturating cystourethrogram.
If there is a narrowing of the urethra, it would require dilatation. A hypertrophy of the bladder neck muscle would require surgery.
Neurogenic bladder will require rehabilitation

Thank you! Is there a chance the urine is going up to the kidneys instead of goong down?

Brief Answer:
Very little chance unless it is a long standing obstruction.

Detailed Answer:
Hello,

The condition is called a vesico-ureteric reflux.
It is usually congenital and occurs in children. You would have been very sick as a child with frequent urinary infections, if you had this problem.
There can be a secondary VU reflux in which valvular mechanism is intact and healthy to start with but becomes overwhelmed by raised bladder pressures associated with obstruction, which distorts the ureterovesical junction. The obstructions may be anatomical or functional .Anatomical: Posterior urethral valves; urethral or meatal stenosis.
Functional: Bladder instability, neurogenic bladder and non-neurogenic neurogenic bladder. Urinary tract infections may cause reflux due to the elevated pressures associated with inflammation.
So in you the flow of urine back into the kidneys is possible only secondary to any obstruction in the flow in the normal direction as I had described before.

Brief Answer:
Frequent urination is good

Detailed Answer:
It is important that you evacuate the bladder frequently to reduce reflux into the kidneys which can hurt them.

How do i make that happen?
doctor
Answered by Dr. Noble Zachariah (1 hour later)
Brief Answer:
Shorten the interval between urination.

Detailed Answer:

Symptoms of underactive bladder:
Full bladder and possibly urine leakage
Inability to tell when the bladder is full
Problems starting to urinate or emptying all the urine from the bladder
Urinary retention
Keep a urination diary. If you are normally in the habit of voiding every 4 hours, shorten the time to 3 hours, so that your bladder does not get distended.
You can also apply some pressure in the lower abdomen, helping the bladder to empty itself.
If these fail, there are other modes of treatment and medicines.
Please let me know your symptoms on the basis of which the diagnosis was made.

Brief Answer:
Confirmed neurogenic bladder requires treatment options below

Detailed Answer:
If the diagnosis was made on the basis of Urodynamic studies (bladder function tests) conducted to measure bladder capacity, bladder pressures, the flow of urine, and bladder emptying, a cystoscopy and skull, spine, and urinary tract examined with X-rays, computed tomography (CT), and magnetic resonance imaging (MRI) and a neurologist consultation and you have one of the following conditions
stroke
Parkinson’s disease
multiple sclerosis
spinal cord injuries
spinal surgeries
trauma/accidents
diseases that affect the nervous system
central nervous system tumors
spinal congenital (present at birth) abnormalities
heavy metal poisoning
diseases such as syphilis, diabetes, and polio
then neurogenic bladder is confirmed.
An obstruction at the neck of the bladder or a stricture of the urethra is commoner and easier to treat than a neurogenic bladder.
There are five types of treatment for neurogenic bladder: physical-psychological, electrical-stimulatory, drug therapy, intermittent self-catheterization, and surgery. The correct treatment is determined by symptoms, type, and extent of nerve damage
Since you do not have urgency, electrical stimulation therapy, intermittent self-catheterization and surgically weakening the sphincter muscles by a procedure called sphincter resection, which removes a portion of the muscle, or complete removal(sphincterotomy) are the options available.
