Question: Hi I am a 40 year old female for past six weeks or so have been experiencing strange rectal pressure about a month ago went to gp she performed a rectal exam and believed I had a fissure she also said that my rectal muscles were abnormally tight she gave me suppositories the pressure still remains after using the suppositories and I am know paranoid that I have rectal cancer my question is can I have a fissure and have no pain related with bm and can this strange sensation be related to tight rectal muscles I have no abdominal pain or blood

i read your question and i understand your concern. Strange rectal pressure for a month examination revealing anal fissure and tightening of anal sphincter explains the pressure. Suppositories to keep the stool soft, warm sitz and saline baths, topical vasodilators creams over the anus area should help with the dilation. Some times when fissures get old, they become anal tags and then pain might not be a problem and tightness and pressure is still there, so yes it is possible to have no pain and have a fissure / tag. Do as directed, do home remedies to keep the bowel movements soft and use sitz bath , drink adequate fluids and bulk forming stools diet like diet rich in fiber and hope for the best. And no it is not cancer at all so no worries for that.

So the rectal pressure which feels like I need to have a bm is related to muscle spasms which may remain after fissure is healed I also notice that stress makes the pressure worse is that common

Stress, can reduce bowel peristalsis and lead to rectal fullness and pressure. And yes anal fissure leading to tags can still narrow the anal aperture and lead to pressure and fullness. Management is aiming for controlling constipation and diet rich in fiber with adequate fluids intake , laxatives and good bowel habits.
