Question: Why do i keep getting chest discomfort

Hi , i am 26 years old and had been diagnosed with GERD . I currently taking Omeprazole 20 mg and Metronidazole 500 mg for it. I been having chest pain on my left and right chest and it radiate to my left shoulder , back and neck but never to my lower jaw. Today when i was walking around my neighbors with my fiance then i feel like a slight discomfort on both sides of my lower jaw and left arm, left shoulder. The pain lasted for about 5 minutes then stop. I read some article on the internet and the symptoms are similar to a heart attack. I still having pain on my chest, left arm and shoulder but only last for a few second. I had been to the ER and the Doctor did do ECG, blood work, chest x-ray and they all came back normal. I'm 26 years old, weight 146 pounds and 5 ' 7 . I'm a smoker but no family members with heart problem. My question is these symptoms could be a heart related problem??

Since you are a smoker and the radiation of pain you are describing occurs in cardiac issues, I would like to know a few more details to help you better:

1. Since when are you having this problem?
2. How was you diagnosed as having GERD? On basis of endoscopy or these symptoms?
3. Do you have breathlessness or chest pain on walking?
4. Do you also consume alcohol?
5. Attach the ECG.
6. Do you develop sweating, palpitations with pain?
7. When do you develop such pain? on exertion or all the time?
8. Any pain on movement?

Hi Dr XXXXXXX Varshney and thanks for the answer

I was experiencing this problem since this morning and now its 11:05 at night. I went to a hospital near to my house last week because of the pain on my chest and they did ultrasound on my gallbladder and he said i having GERD and stomach infection. He prescripted me Metronidazole and Omeprazole for it. I dont feel breathlessness and chest pain while walking, just a discomfort on my arm but i do get chest pain, shoulder and arm pain all the time even while im sitting. I never sweating as well. I do have pain on the chest if i move to the left or bench forward sometime. I do comsome alcohol excessively the past few weeks but i stopped for 2 days.

Dear Friend,
Since you have an acute history, this pain can be due to:
1. Acute Cholecystitis
2. Hyperacidity

However, we should rule out Cardiac problem, even if your history is not suggestive of that.

I would advise you following:

1. To get a fresh ECG again and Cardiac enzymes, this would rule out any cardiac problem.
2. To take a combination of Pantoprazole and Domperidone once a day, this is going to help you with GERD more then Omeprazole alone.
3. If you have any loose motions or stomach pain, then only Metronidazole is required else not.
4. In case you have persistent pain abdomen, and you had consumed Alcohol recently, you should get a work up for Acute Pancreatitis.

However, in your situation, i would say that get an ECG again and take these medications, if you have persistent pain, then go for further workup.

Also, this is important to mention here that DO NOT take Alcohol since you are on Metronidazole, else you would get a severe reaction called as "Dilsulfram Like Reaction"
