Brief Answer:
It may be a sign of fungal infection.

Detailed Answer:
Hi, it is a wise step to ask your health related query here.
As you told, you are having this problem since many months and it mainly affects your tip of penis and foreskin.

These conditions points towards possibility of a chronic infection like fungal infection. Fungal infection ( candidiasis) of penis typically affects tip of the penis and foreskin. It causes minor cracks and loss of elasticity in foreskin. As a result there is difficulty in retraction of foreskin during sex and bruise and inflamation of the area during sex.

You may need to be prescribed an antifungal cream like Clotrimazole for applying locally and some antifungal oral medicine like fluconazol for a period. Your partner should also be treated simultaneously to avoid cross infection. You should also get your blood sugar tested to rule out diabetes.

Hope this answers your query. I wish you a better health.
