What Causes Blood Clots In Urine?

Question: Dear Sir,


Right now I am facing with multiple health issues.
last month my heart beat suddenly went upto around 250 while working in office.

after few days faced problem while urine. and last week again
i found thete is a blood clot in my urine and pain while urine. also my sugar level before breakfast is approx 100 and after around 160.

Also around 8 year backs doctor told me that i have varicocile in my left testis.

I am in fear, if i have any major disease ?
I am attaching all the recent reports as well.
please suggest me if any other tests required ?

I've gone through your reports and query and understand your concern..

First of all don't panic!!

None of these reports have shown anything too much concerning..

Don't just go for investigations randomly as it'll bring more and more stress and worry as you're experiencing now..

I'm summarizing the conditions below, so request you to go through them really carefully and make a note of them if necessary and please don't presume things..

To summarize your case history as per your description as well as these reports accordingly.. i.e. Your main concern is Urological- at first you had pain during micturition followed by some blood clots in urine, then on 23/04/16 morning, you underwent for USG whole abdomen which showed some area of focus in the left kidney but was in-conclusive so you underwent for Spiral CT-KUB on the same day which showed that the Stone(4 mm) was passed from left kidney to the left ureter and after one month, you again underwent CECT-KUB which doesn't show any more calculus in kidney and ureter , however there was some homogenous contents in side the bladder.. this means that the Calculus most probably traveled all the way down to your bladder now..

Additionally you've mentioned about the past history of varicocele in left testis 8 years back..

Your blood reports are all within normal limits however the urine microscopy has shown presence of calcium Oxalate which is indicative of the stone in your urine stream only..

For further assessment , you need to schedule a visit to Urologist and get thoroughly examined physically which will also give you a clarification if there is any element of varicocele is there or not and further course of action for the residual stone in side the bladder..


Second point of concern is your sudden increase in heart-rate , there are several queries regarding-

1. How did you measure a heart-rate of 250 bpm at office ?

2. Do you have any past/present history of any heart condition? was it the first time ?

3. Are you on any cardiac medications ?

4. Was there any ECG taken at that time ? if yes, please upload the complete ECG rhythm strip(not the report, the rhythm)..

5. Your 2D-Echo didn't show anything significant, so not to worry..

6. I believe you must've visited a cardiologist, if not then I'd strongly recommend you to visit one immediately for a further check-up as well as investigations- which may include a Stress test, 24 hr Holter monitoring for further evaluation in this regard..


Thirdly, you've mentioned your pre and post meal blood sugar, are you a Diabetic ? Do you have any family history of diabetes ? How long back was this reading taken ? Did you use the finger prick method with glucometer to check ?

If yes , then you can go for Fasting and 2 hrs post prandial blood glucose level along with HbA1C measurement for further clarifications..

Meanwhile start drinking a lot of water(~3.5 ltr per day) , it'll help to keep your kidneys cleansed and keep your urine diluted.. avoid drinking tea..discuss with the urologist regarding starting of any urinary alkalizer for symptomatic relief..

Dear Sir,

First of all thank you for clearing the doubts.
I am attaching my ECG reports as well.

Also 3 things I missing to share ,

1) Sometimes I fill pain in anal while sitting from inside , So need to check with Urologist for the same ?

2) I have noticing some time red spots in my tongue & inside mouth and sometime my tongue become bulky and white and feeling uncomfortable while opening the mouth.

3) Having Fatty liver Grade 2. I lost around 5.5 Kg weight in last 1 month

And below are my answer for your futher questions.

Ans : I was admitted to hospital in emergency. At 1 time I thought I will be no more.

2. Do you have any past/present history of any heart condition? was it the first time ?

Ans : Previously this time of situation I was facing once in a year or 2. last time this happened when I was playing cricket around 10 months back.

3. Are you on any cardiac medications ?
Ans : No, but my father was Cardiac patient and have Prostate problem. MY mother was also Cancer Patient(ovarien).

4. Was there any ECG taken at that time ? if yes, please upload the complete ECG rhythm strip(not the report, the rhythm)..
Ans : Yes. I attached the same.

5. Thirdly, you've mentioned your pre and post meal blood sugar, are you a Diabetic ? Do you have any family history of diabetes ? How long back was this reading taken ? Did you use the finger prick method with glucometer to check ?

Ans : Current report saying my sugar level of today 100/154 (before/after meal)
but 6 months back I checked my complete profile test, which was ok. only vitamin D deficiency and SGOT/SGPT was on higher side. My mother was diabetic patient.

Thanks for following up, yes I've gone through the ECG , it's just showing sinus tachycardia(regular fast rhythm of the heart), so need to worry about about that..

Now you need to understand that any kind of stress(even anxiety), physical exertion can make the heart pump faster, that's normal physiological response, so that previous incident while playing cricket might be just normal, but as I've already mentioned, to rule out any other conduction anomaly those other tests are essential, so just discuss them with your cardiologist in details..


Also, since you've mentioned grade II fatty liver, inform your cardiologist the same along with liver function test and lipid profile to decide if you need any lipid lowering medications or not..

Loosing weight is good in your case but it has to done under supervision, so Please get in touch with a dietitian for a complete diet chart preparation so that you can follow it daily..beside them, once you get in touch with the cardiologist ask if you can be cleared for physical activities, if allowed, then can plan for mild to moderate physical activities..


Now please focus on the symptoms which are more concerning, so that it'll be easy for the treating doctor also to remain focused on the priorities first.. In this case the rectal pain can be muscular, can be even bony so bring that up to the urologist only when the more concerning part(stone) is over, so that the urologist can guide you accordingly..
