What Causes A Dull Ache In The Upper Right Quadrant Area?

default
Posted on Fri, 21 Oct 2016
Question: Hi, I'm a 34 yr old female and my symptoms started in march this year. Initially I had a dull ache intermittently in my upper right quadrant. A few weeks later I also noticed my stools were paler and more watery than usual. I looked these symptoms up and it advised I see my GP. I was extremely concerned about pancreatic cancer but was assured it was very unlikely at my age. I was referred to a gastroenteroligist after some convincing. He was a little more sympathetic and also said pancreatic cancer was a very unlikely possibility. He ordered cbc, liver function test, iron studies, stool analysis for steathorrea, fecal elastase test. Colonoscopy , gastroscopy, mri of pancreas without contrast and mrcp and also ultrasound. Every test came back normal and this reasurred me. I did lose some weight due to the diahorrea but the gastroenterologist said he was reassured from my tests results that it was not anything serious. Since my last appointment with him I have had 2 new symptoms occur. Frequent short spells of nausea, sometimes only lasting 5 minutes, Half an hour at the most. More worryingly low back pain, which is also intermittent but at times severe, which sometimes radiates into my legs. I am still very concerned about pancreatic cancer and am wondering if they could have missed it as my mri was without contrast? The nausea has been for over a month and the back pain for 2 or 3 weeks. Should I be worried or is it likely to be caused by something else. I also suffer mild health anxiety. I would greatly value your professional opinion, many thanks. XXXXXXX

Brief Answer:
Pancreatic Cancer is extremely unlikely

Detailed Answer:
Hello Madam

I can certainly understand your concern.

A normal MRI(even without contrast) in association with normal blood tests will literally rule out pancreatic Cancer

Pancreatic Cancer is usually a disease of elderly men who smoke and drink alcohol.

So Pancreatic Cancer is absolutely rules out in your case. It's clinical presentation is a rapidly deteriorating course with high jaundice.

Hence you can be reassured .

Many of your symptoms could be related to underlying anxiety.A short course of anxiolytic drugs will improve your symptoms to a large extent.

I generally prescribe oral pills of Tab Nexito plus once daily at bedtime and Tab Pantoprazole 40 mg on empty stomach at 7 am daily for 3 months to my patients with anxiety disorder.

Please check with your physician if he shares my view and if can prescribe these pills to you.
Thank you.
