Hello!

Welcome and thank you for asking on HCM!

I carefully passed through your question and would explain that your symptoms do not seem to be related to any cardiac disorder.

Anxiety could be the cause of these troubles.

Anyway, I would recommend consulting with your attending physician for a careful physical exam and some tests to exclude other possible causes underlying your symptoms:

- a cardiac ultrasound
- an ambulatory 24-48 hours ECG monitoring (Holter) to examine your heart rhythm trends for a prolonged time
- complete blood count for anemia
- thyroid hormone levels for thyroid dysfunction
- blood electrolytes for possible imbalance.

If all these tests result normal, you should consider anxiety as the main possible cause of your symptoms.

Meanwhile, I recommend avoiding caffeine intake or coffee, which can trigger you symptoms.

Hope to have been helpful!

Feel free to ask me again whenever you need!

Kind regards,

Dr. Iliri

Hello again!

I understand your concern.

I should say that gastro-esophgeal reflux could also mimic all this clinical scenario.

That is why, I would recommend taking Omeprazole or Zantac before sleep. Avoiding large meals in the evening or eat your last meal early (before eight o clock) could also help.

But, if your symptoms are not relieve by these measures, I would recommend consulting with your doctor and performing all the above mentioned tests.

Hope to have been helpful!

Wishing all the best,

Dr. Iliri

Hello again!

Some ectopic heart beats are quite normal in every healthy person. They are related to emotional stress or physical activity.

Gastro-esophageal reflux could also cause some extra heart beats from the discomfort.

Anyway, it would not explain your head heaviness. Anxiety or low blood pressure could be the cause of noise and head heaviness.

That is why I would recommend you to closely monitor your blood pressure values too.

The 24 hours ECG monitoring will definitely help to exclude any cardiac disorders.

Hope to have been helpful!

Wishing all the best,

Hello again!

Thank you for the additional information!

Coming to this point, you should know that all your symptoms (difficulty falling asleep, extra heart beats, etc.) seem to indicate anxiety.

The fact that Trika (which contains alprazolam) helped improve your situation, is another argument in favor of anxiety as the main cause underlying your symptoms.

That is why I would recommend consulting with a specialist of this field (psychotherapist, psychiatrist) to help improve your situation.

Meanwhile, I would recommend trying Melatonine 5 mg before sleep, to help with your sleeping problems.

An antidepressant may be needed, but you should consult with a specialist of this field about that. You should know that Trika (like other benzodiazepines) are not recommended for a long term treatment, because they can cause addiction and tolerance and thus exacerbate your situation.

Avoiding caffeine intake and performing a lot of physical activity (walking, Yoga, aerobics, recreational sports, etc.) can improve your situation. You should know that physical activity is very necessary for a healthy mind and body.

Hope to have been helpful to you!

Wishing good health,

Dr. Iliri
