What Are The Symptoms Of Acid Reflux?

This message need to be read in details
A 9 months nightmare problem that ruined my life.
Started in july 2014, in Ramdan and one day during iftar i startaed experiencing phlegum and cough after first piece of food and even when running and some other activities. I thought it was something like some sensitivity that will go alone. after 2-3 month's i started suffering more and more so i decided to go to an ENT. he described some virus medicines and antibiotics but in this week hoarseness and phlegum and cough increased more so i went to other ENT who described nexium 40 mg once a daily and some nasal medicines. After 3 weeks i did not really feel much better we can say 10% better throat clearing and other but with new symptomps as something caught in throat and chest pain. Went to him many times to check if there is other problem rather than reflux he said to continue and it takes time. 1 month later ( 2 months from the beginning ) i started suffering food stucking in throat, liquid back in throat while eating, eating and all things. went to a gastroenterologist who said we will increase nexium to twice 40 mg daily and see after 3 weeks to do an endoscopy. After 3 weeks same symptoms, lump in throat, food stuck, chest pain, upper stomach pain, phlegum, 24 hour of life ruin. did the endoscopy and results clear with no signs of reflux. Continued with 40 mg controloc twice daily for 5 months now went for other doctors giving same diagnosis and in the 5 months the symptoms change daily 24 hours with: chronic lump in throat, food stucking in throat and chest, stomach pain, sore throats, phlegum, post nasal drip, liquid in throat feeling hot, upper stomach pain, and others I forget because of the countless symptoms. Esophageal manometry: 100% normal PH test: Mild reflux highly correlated with symptoms. ENT report: Signs on throat fore reflux. Now 9 months, i started with 3 symptoms, after months of trying maximum dosage of medicines symptoms actually increase !!
Notes:- sometimes food doesnt really seem to matter.
sometimes throat seems to be clear and after even drinking 1 ml of water throat feels like so tight !
Stomach feels fulled with water so quickly.
- Elevated the bed for the last 9 months.
- did not eat acidic or fatty foods for 5 months
- last meal eaten before 3 hours from going to bed
- don't lay down frequently after eating.
there is other details may be i have forgotten due to the long lasting irritating period.
TEST: NORMAL MANOMETRY, MILD REFLUX ( THE DAY OF THE PROCEDURE WAS THE LEAST SUFFERING DAILY, NORMAL 2 ENDOSCOPY, 1 SHOWED MILD GASTRITIS AND H PYLORI, NEGATIVE FOR ESOPHLANIC ESOPHAGITIS, THROAT ENDOSCOPY SHOWED REFLUX SYMPTOPMS)

Hi,

I have gone through your medical history carefully and am sorry for the situation you are in.

Based on your clinical symptoms and reports of tests you did, it seems that all your symptoms are related to acid reflux. However, in order to rule out any possible mass located on nasopharynx, I advise to discuss with your doctor about running ct-scan or MRI of nasopharynx.

Endoscopic Ultrasound (EUS) is also recommended together with neurological examination of upper GI organs to rule out neurological problems like paresis for example.

For the moment, I advise to follow the lifestyle and diet regimen as you are doing now. Discuss with your doctor to change therapy from omeprazole to rabeprazole and to increase the dose. Antacids are also recommended to use after meal.

According to your location (Egypt), I also advise to run stool examination for ova/parasites as they are prevalent in Egypt and might be possible cause to your symptoms.

Thank you for your replay and your detailed reading !
Do you think really these are associated with the symptoms?
And did you see a case like this before ?
24 hours non stop pains between all I said before.
and if it is only reflux, why it is not solved by all these high dosage medecines?
and why does food does not matter acidic or not.
The point is that not only the controloc is not doing anything, the nasal medecines also are not doing anything in the phlegum issues !! 9 monthes is really too much 9 monthes 30 days 24 hours !
i will give you a one day example !
today i walk up with phlegm and stomach pain
after the controloc 40 and ate breakfast of some cereals and light milk and depsite taking 2 tsp gaviscon advane then throat pain for 2-3 hours then ate an apple still throat pain. ate lunch grilled chicken and boiled pasta then chest pain and phlegum and throat and stomach pain despite the gaviscon advance. 40mg control again and then light cheese and bread for dinner and gaviscon advance and soo soo bad throat pain !! this is every day and more in the 9 monthes and also the food stucking in throat and chest.

1. Yes, acid reflux is associated with the symptoms you are currently experiencing.

2. My advice was based on cases I see every day in my clinic. I admit that there are only few (1 or 2) cases being very resistant to the medications although of high dose.

3. If I was your caring doctor, after evaluating your case, I'd advise to run the above-mentioned examinations to determine the final diagnosis.

4. Stress is also an important factor: stress will cause hyperacidity which will aggravate reflux symptoms.

what about upper abdominal pain that does not releif by many sppoons of acid reflux medecines ? and walking up with lower stomach pain ?

Hi back,

Sorry for the late reply due to my urgent clinic duties.

With regards to upper abdominal pain, that is due to acid reflux, also known as gastric pain.

The lower stomach pain you are feeling might be due to:
- intestinal spasms especially if after consuming a specific food/drink
- uro-genital problems: if you are having other uro-genital complains apart the pain: discharge? burning during urination?
- if it is aggravating during walking on, then, it might be related to abdominal muscles and not to internal organs. Try to take an over the counter pain killer and see if the pain will be alleviated.

yes i am having also 2-3 years burning in urination and pains in the urinary areas, and in the last 2-3 weeks pain around the butthole. sorry for all these questions but my case is irritating.

As there are various causes to your clinical symptoms, your additional data are indicative for possible urinary tract infection. Urine analysis, urine culture and abdominal ultrasound will help determine the right diagnosis and specific treatment.

For the moment, I advise to take magnesium citrate to get the burning sensation relieved.

Increase water intake to help improve your actual symptoms.

With regards to burning in the butthole (anus) it might be due to possible candida or parasitic infection. Examination of stool for ova / parasites and culture of feces for candida are necessary to provide the final diagnosis.
