Question: Regarding STDs

my wife and I decided to bring in a third person to bed with us
she did blow job on him and me ( no condom )
and then he had protected sex with my wife and I had unprotected sex with my wife .

are there chances of catching any std through this encounter with
the other guy if he had an std?

Brief Answer:
Both protected & unprotected oral sex with 3rd person can possibly transmit

Detailed Answer:
Hello. Thank you for writing to us.

I have gone through your query and I have understood it.

I would like to clarify whether unprotected oral sex with third person was followed by ejaculation into her mouth? Oral sex carries a risk of her getting oro-labial herpes from oro-genital sexual contact. Presence of visible sores of a particular STD e.g syphilis or herpes can transit these STDs as well as increasing the risk for transmission of Hiv. Ejaculation into mouth or urethral discharge also increases the risk for other STDs i.e throat gonorrhea, chlamydia, syphilis including Hiv for the partner who is giving oral sex.
However, assuming that he had no visible genital sores or urethral discharge oral herpes is the only likely possibility.

As far as protected sex with third person, it does carries a risk for transmission of Herpes and HPV. The reason being a condom covers only a portion of the shaft of penis and leaves a lot of genital skin exposed for contact during sexual intercourse e.g base of shaft, pubic skin and scrotum. Risk for transmission of herpes is more if he had an active ongoing episode or sores of herpes, less otherwise. Protected sex with a male is safe in terms of other STDs including Hiv apart from herpes and Hpv.

There was only oral sex that my wife performed on both of us
no ejaculation into her mouth. As far as Herpes , I am not concerned about cause we already have 1 and 2 and also HPV too.

1.So my main concern was HIV transmission , Gonorrhea or chlamydia or Trich mainly.

2. Reason is my wife says she is itching in the vagina after our little experiment.
As soon as we had this encounter she got a herpes outbreak on her lip.
She told me she was really nervous about this experience .
And I told her that she is itching because she has probably genital outbreak.

I did not get an out an outbreak although I was kissing my wife with her sore
and she also performed oral sex on me with her cold sore.

Brief Answer:
There is a very low but definite risk of Hiv through oral sex

Detailed Answer:
Hi.

Oral sex as such carries a very low risk for Hiv but it is not an absolute Zero. Ejaculation into mouth does increase the risk as does presence of symptoms of another STD like urethral discharge or genital sores in the partner receiving oral sex. The reason being that there will be more of infecting secretions that would also contain a higher viral load.
Gonorrhea, chlamydia can transmit from urethra to throat/pharynx through oral sex. But before I give my opinion I would like to know if she has a vaginal discharge or burning sensation on passing urine?

Vaginal itching without visible sores may be either candidal Or a bacterial vaginosis which are not actually STDs.

Brief Answer:
Bacterial vaginosis is likely; Test for STD's at appropriate time

Detailed Answer:
Hi.

Most likely she is suffering from bacterial vaginosis, which is a condition due to alteration of vaginal bacterial flora. It is not an STD.
A new partner Or having multiple sexual partenrs can be one of the reason behind vaginosis.
If I was the treating doctor I would treat her empirically with metronidazole tablet 400mg thrice daily for 5 days.
An STD test just 8 days after a particular sexual contact can probably help diagnose only some but not all STDs e.g a patient can test positive for chlamydia, gonorrhea by a swab test/ NAAT test (Nucleic acid amplification test) within a few days after infection. However an 8th day test for STDs would reveal nothing as far as other STD's are concerned i.e syphilis, Hiv.
Ideally a negative swab test for gonorrhea and chlamydia is taken to be conclusive at 2 weeks. Therefore in those who are strongly suspected to have any of these STD's should be retested at 2 weeks even if an earlier swab test is negative.
Anyways since she had condom protected sex with third individual therefore it is unlikely that she would test positive for gonorrhea Or chlamydia.
As far as timing of STD testing is concerned I suggest that she gets screened for HIV and syphilis at 4 weeks and then again at 12 weeks.
