Uderweight and having poor appetite. Started vomiting while eating. Help?

My 12 year old son is very small and underweight (48. Lbs) He has a very poor appetite. I have tried a few appetite stimulants, but he tends to vomit them because of the bad taste. He is a vegetarian (no meat/fish or eggs) in the diet. He has no sickness
He is about to start secondary school later this years and I really would like to help him gain weight. Recently he started vomitting when he eats certain snacks. Please advise.

Thanks for contacting XXXXXXX and Ayurveda with your concern, and going by the history you have provided,following issues,along with preventive and curative aspects are discussed:
1. Although weight is mentioned that is roughly 21.82Kg which is less for 12 years,but you have not mentioned the height as said in your history (he is very small),which will also help to assess the Body mass index (BMI) of the child.
2. have you completed his Immunization schedule till 12 years?
3. any deprivation in feeding or personal care(was he exclusively breast feed or Extra nutrition was provided from outside?) or health related issues of mother during pregnancy (she herself being under nourished and stunted??),
4. any associated cognitive dysfunction?
5. have you checked with your healthcare provider regarding Hemoglobin,cough,low grade fever? Are tonsils enlarged? Or history of eating mud,flour,chalk or any inedible substance? History of allergy and acid reflux is there: due to acid reflux he might be suffering from vomiting (apart from bad taste) resulting in not absorbing nutrients properly and not gaining weight!
i. encourage outdoor games and exercises like running,swimming,Yogasanas,Surya-namaskar.
ii. take protein XXXXXXX diet (legumes,sprouted pulses/cereals,milk with protein powder (which are required for manufacturing of antibodies)
iii.Take vitamin A,B,C,E in diet which helps in strengthening the immune system.
iv. self expressed juice of white onion 6ml with honey in morning and good sleep after meals (he will definitely gain weight)
1.first deworming is essential: make powder of raw mango seed and give 250mg with yogurt or water empty stomach in morning for three days(AVOID: cucumber,long cucumber,raw fruits, potato,meat,sugar,sour foods,milk,jaggery,clarified butter for 15 days) and give walnut,PINEAPPLE,peach
2.there after improve digestion : avoid: heavy,slimy food (initially)along with fetid odor and unpleasant sight and give old rice (1 year old but not diseased), green gram,fresh green vegetables, banana, pomegranate, garlic,ginger,raisins,black pepper,clarified butter,butter milk, regular brushing of teeth and tongue cleaning, bathing, lemon,salt,mint,morning walk
..Syrup Liv.52 or Syrup Livomyn 2tsf thrice a day,(1/2 hour after meals)
.Hingvashtak churna 1 tsf dissolved in 1 glass of lukewarm water and half lemon squeezed and give him to drink spoon by spoon every 5 minutes (1/2 hour before meals)
XXXXXXX XXXXXXX ras half tablet with honey and thereafter take 10ml of Kaumaryasav with equal amount of water (after meals) twice a day.
