Suggest Treatment For Swollen Lesion On The Genitals

Question: I'm XXXXXXX about 3 days ago I was having sex with my girlfriend. for a second it felt like something bit me down in my genital area by the base of my penis shaft on my scrotum. about an hour or 2 later that area on my shaft got swollen and very itchy to the point wear it felt kinda painful. then my lymph node started swelling up to the point that it was painful and difficult to walk. I was able to reduce the swelling by massaging it. it's now been about 4hrs. later that evening I go to the restroom n take a closer inspection of the inflamed area.. I notice a tiny hole that's very hard to see like the tip of a sharpens pencil. I gnite it. a little later I look at it again and that tiny area by the hole is turning grey. I go to sleep. then next day my lymph nodes have reduced in size but the the tiny area of skin that's turning grey has turned black and has gotten bigger. still itchy around it... but swelling has him down . now today I noticed the black area has started to become a scab and is breaking off. I put antibiotic cream on it to prevent it from stinging to bad which it now does since its now basically a open sore or leasion. I will attach a current photo is this something I need immediate attention to or a std? I would like your thoughts please. thank you for your time.. sincerely XXXXXXX

At this moment you may just use an antibiotic cream/ointment, 2x daily, for this sore.
I would like to know more about your sexual history. Do you have a single or multiple sexual partners? Is there a history of any STI in your girlfriend?and does she have or have had other sexual partners?
Any genital sore in a sexually active male demands consideration of an STI.
I would suggest basic STD screening for Syphilis, herpes genitalis and HIV. These tests should be repeated at 12 weeks

No multiple partners. I accidentally sent this question in twice the other doctor recommended I get checked for herpes n get a swab culture test thing done as well while the black skin is off fresh I'll be looking into a doctor tomorrow/ today n get that done at a urgent clinic so I can get it done quickly

Yes. I totally agree. STD screening (blood tests, Swabs and Cultures etc ) should be considered in any case of a sexually active individual who presents with a genital sore.

So went to the doctor . she said it didn't look like any type of herpes lesion she's ever seen but similar to a syphilis chancre but doesn't believe that it is because she a chancre is usually bigger deeper yellowish and painless while mine was smaller shallower pink and painful (it stung a lot) and that it may have been a friction burn of some sort though and gave me antibiotic ointment for it and penicillin shots just in case even though she's never heard of a std that had quite the same symptoms the hole turning grey then black n spreading like decaying skin thing... and had a full panel std screen test and will have results soon 👍

Yes. Even I considered the possibility of syphilis if you can refer back to my earlier reply. Although syphilitic chancre is painless and feels firm on palpation because it has induration at base but it still need to be excluded in a patient with a genital sore as pain is a subjective symptom and patients can have some pain and moreover a chancre may become painful if it develops secondary infection. Your description of how the lesion started and how it changed over time is a clue to the diagnosis whether it is a chancre or not but clinical morphology and sexual history coupled with lab tests are also essential in confirming or ruling out a diagnosis.
Anyways I ll wait for your test reports.

You mean to say that you tested positive for Herpes simplex type 1 virus?

Herpes Simples type 1 usually cause cold sores (oro-labia herpes) and it less commonly causes sores of genital herpes. Herpes simplex type 2 is more commonly a cause for genital sores of herpes.
Nevertheless both types of herpes viruses (type 1 and type 2) can cause either oral or genital sores.

Yep it's was type 1 virus quite weird symptoms but the doc said the swab tested positive for type 1 virus

Genital infection with Hsv type 1 is less common as compared to type 2 Hsv. Moreover, genital Hsv type 1 is less symptomatic as compared to genital Hsv type 2 infection. Hsv type 1 episodes are milder and may even go unnoticed. Genital sores due to Hsv type 1 are also less frequent as compared to Hsv type 2.
Treatment is the same for both types of genital Hsv infection. An episode of genital sores is typically treated with oral antivirals i.e either acyclovir or valacyclovir.
I suggest you to always have protected sex with a condom.
