Question: 40 male patient diagnosed with Subacuate Myocardial Infarction.RCA90%,LAD 99% and circumflex-70-90%.TVD detected.placed 2 stent.no past medical history.what could be the resaon for CAD in no history of diabetes or hypertension/dyslipidemia.

Hello,
Heart disease results from either genetic or environmental factors. Sometimes, genetic factors are very strong and it itself may result in blockages. Family history may be present in such individuals, but not always. Is he obese, smoker, alcoholic, or having other diseases in the body, may lead to this. Smoking is especially important, as it can cause attack in very early age groups. So should quit if having smoking, alcohol or drugs. If none is present, then some individuals have genetic defect and their has increased tendency to form clots in the blood. So he should evaluated for the same. For this, he should undergo serum homocysteine levels, factor 5 laiden, protein C & S test.
To me the likely cause appears to be strong genetic factors, and may be smoking if smoker. Fortunately, his heart function is normal, so strong precautions to be taken. Medicine prescribed is good and covering from all the sides. So there shouldn't be default in medicine at any cost.
He should have a healthy lifestyle like avoiding fatty, oily and high calorie diet. Have low salt diet and monitor blood pressure regularly, and bp shouldn't be persistently more than 140/90 mmhg. Regular exercises like brisk walking, jogging according his capacity atleast 30 min a day and 5 days a week. Lots of green leafy vegetables, fruits, fish once or twice a week, avoid meat. Avoid smoking and alcohol if any. There shouldn't abdominal fat deposition or obesity. Hope this helps you and get back if you have any doubts.
