Suggest Treatment For Skin Allergy On Wrist In A Child
Question: Hi,
My 7 year old son has got some kind of allergy around both his wrists. The skin turns red all of a sudden and he complains of burning and pain. It does not stay all the time, but is on and off quite frequently. It is usually seen during the evening and night time. The skin around the area has turned dark in color and has become quite rough when touched. It's been 2-3 weeks now.
Can you please let me know what exactly is the problem?

From your description, the child seems to have developed a sort of a dermatitis. However, I would request you to kindly click a few Images of the affected area and post it to me either by uploading at "Reports Section" Or alternatively you may mail me the Images as an attachment at the following email ID: YYYY@YYYY with the subject line as Attention:Dr. Kakkar. Use a moisturiser and a moderately potent topical steroid for a week

He has developed dermatitis which is most likely secondary to extreme dryness of skin.
I would suggest a moisturiser e.g cetaphil moisturising cream after mixing with an equal proportion (i.e 1:1) of a moderately potent topical steroid cream e.g mometasone furoate 0.1% cream (Elocon cream), , to be applied twice daily. Use these for a week. Thereafter, just continue with the moisturiser for a longer duration.
These are prescription medications and I suggest that you use them under supervision of a doctor in your vicinity.
Avoid frequent unnecessary hand washing with soaps as soaps exacerbate dryness of skin. Instead use a gentle cleanser for hand wash e.g cetaphil cleansing lotion, twice daily.
You may also give him a teaspoon of cetrizine syp 5 mg/ 5ml once daily for symptomatic relief from itching.

He does not have any itching or swelling on the hands. It is just that the allergy is coming on and off. When it is on, the skin turns red in color and he complains of pain and burning sensation. When it is off, there is no pain, but the skin remains in the dark colored state. I wanted to know if there would be any inflammation or infection from inside the skin.

Though, you may stop the steroid cream after a few days, however, I suggest you to continue the moisturiser, even afterwards the skin condition has resolved, specially in dry and cold weather of winters. Coconut oil would not be of much help.
