Question: Hi, I have had a missed miscarriage in April 2016 at 7 weeks with no fetal pole and yolk sac seen.I am pregnant again. Triplets reduced to singleton. I conceived last december with iui. My lmp is 22nov2016. I was on ecospirin 75 mg for atleast last three month before IUI. On 3rd jan17, three sacs were seen. On 10th, two heartbeats were recorded and the other one just had fetal pole. This time I was given heparin 40mg alternate days and 150mg ecosprin. I was also given susten 400mg twice and duphaston 10mg twice. Then on 24th january 2017 only one twin heart beat was recorded. The other twin had stopped growing on 7 weeks 6 days. Doctor asked me to take 60mg heparin everyday. Since then I am on heparin every day plus ecosprin 150.however duphaston is stopped. I had NT scan on 14th feb 2017 showing high resistance blood flow in both uterine arteries with an early diastolic notch in left artery. My RI is 0.82 and 0.92 in right and left uterine arteries respectively. My PI is 2.91 and 3.66 in right and left uterine arteries respectively. Since 13th feb i am having severe headache above right eyebrow covering right side of my hindbrain and neck. It comes and goes but does not t subside at all.I thought it is a migraine or gastritis pain but it is there past 6 days. My doctor asked me to start pan-d tablet for seven days and has asked me to take heparin60 on alternate days instead of every day. Please suggest why pain is still there after taking pan d for two days. What else can be done to increase the blood flow to fetus.
Also I am taking All9D tablet as supplement. Please advise me if I can take any calcium tablet also.

Brief Answer:
Headache needs evaluation.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
I have gone through detailed history and all the reports provided. Early diastolic notch that you have can cause a decreased placental circulation that can be helped with heparin. The notch can disappear gradually as the pregnancy progresses. The headache that you have can be due to gastritis weak eyesight stress anxiety raised blood pressure postural or due to cervical causes. All these causes need to be evaluated for proper treatment. The medicines that you are taking now are sufficient to increase the blood flow. Vaginal sildenafil tablet insertion can also improve the blood flow. You can take it after consulting your gynecologist.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Hello Mam, Thanks for answering.
Even after taking 150 mg ecospirin and 60 mg heparin everyday, why high resistance in blood flow is there? My concern is that because of headache, doctor has asked me to take heparin on alternate days. Would it affect the blood flow furthermore?
Also, please let me know if I can take calcium tablet as supplement and what amount I should take. Because the supplement prescribed my doc, tab All9D, has 1 mg L-Methyl folate calcium, 1500 mcg Mecobalamin, 0.5 mg pyridoxal 5' phosphate,200 mg DHA,2000IUVitamin D3.
I was told by someone that I should take atleast 2 gm calcium tablet.

Brief Answer:
You can take calcium supplement.

Detailed Answer:
Hello.
Thanks for writing again.
High resistance blood flow in arteries can be due to raised blood pressure or thin arteries. Decreasing heparin can affect the blood flow to some extent. You can take calcium supplement that contains 1 gm elemental calcium twice a day with milk.
Hope my answer is helpful.
