Brief Answer:
Following advice.

Detailed Answer:
Hi and thank you for asking!
I read your query and understood your concerns.

Your father problems are related to severe heart failure (HF) with an EF of 20 % that cause PH . More over HF is manifested with weight gain, leg edema and constipation.

Heart failure is caused by aging, ischemic heart disease and diabetes. Treating heart failure and precipitation factors will help in reliving his symptoms.

If your father was my patient I would recommend as follows :
- a low sodium diet
- increase diuretic dose (Dytor)
- add spironalactone to your therapy
- use lactulose and other stool softener
- add nitro components in his therapy

Hope this will be of help to relieve his symptoms.
Wish you and your family health!
Dr.Benard
