Question: I have just found a long, thin black worm wiggling in my toilet. The toilet was clean . Could it have been in my urine when I went about three hours earlier even if I had flushed the toilet ? I have no pain.

Parasite infestation of the gastrointestinal tract is a much more likely diagnosis as many a times even during urination a worm can be passed through the anus without one realizing it.

The worm can be isolated and sent for a parasitology identification but I doubt it would be helpful in the laboratory as a diagnosis for your condition as the worm needs to be found from the source and has to remain uncontaminated by its surrounding. Stool for ova and parasites should be advised which will rule out or confirm a parasitic infection. Albendazole or mebendazole can be prescribed if the test confirms worm infestation.

Worm in urine can be due to schistosoma. which is a rare condition called schistomiasis found the in third world countries associated with contaminated water use. Considering your description I doubt its schistomiasis.

What would be the symptoms of such a parasite infestation ?

The worm was about 13cms long - so could it have entered the toilet from the sewer ?

Hello.

In schistosoma infestation a person becomes severely sick much before any worms are passed. Symptoms include fever, urticarial rash, lymphadenopathy, focal neurological deficits especially in acute cases. These symptoms are similar to acute viral or bacterial illness.

Yes, its very much possible that the worm could have entered the toilet from the sewer.
