Brief Answer:
Panic disorder

Detailed Answer:
Hello

Thanks for asking from Health Care Magic

I have read your queries and I can understand your problem. You have anxiety and depressive disorder and which are getting aggravated by poor sleep due to shifts of duty. You were prescribed Seroquel but that drug didn't work and instead made symptoms worse. You are taking Clonazepam 1 mg per day since last 2 years but currently there is no relief. Tried Alprazolam resulted in good relief.

Clonazepam and Alprazolam both are benzodiazepine drugs. Alprazolam is relatively faster acting and have short half life and this is the reason you got relief in panic attacks. There is no need to think about the thing. You can visit your doctor and explain that you are taking clonazepam with no relief and want to take opinion about alprazolam.

Secondly mouth dissolving preparation of clonazepam can also result in almost similar effects. I think there is no problem in asking about mouth dissolving preparation of clonazepam.

As the night shifts and day shifts are causing aggravation of symptoms. As per my opinion drugs that help in normalising melatonin which is a sleep hormone can be tried. Ramelteon is a melatonin agonist. The drug can be taken after night shifts and it will result in normalising sleep rhythm. Ultimately sleep cycle will improve and intensity of anxiety will reduce.

Also clonazepam or alprazolam are the drugs that can help to improve symptoms of anxiety. They are not the drugs for definite treatment. I would recommend you to consult your doctor for anti-anxiety drugs like SSRIs or SNRIs for treatment. These drugs will ultimately help in proper treatment.

Thanks, hope this helps you.

Brief Answer:
SNRI/SSRI

Detailed Answer:
Hello again

SSRIs are selective serotonin reuptake inhibitors. SNRIs are serotonin nor epinephrin reuptake inhibitors. In anxiety and depression levels of serotonin and nor epinephrin are thought to be deficient. These drugs help to improve the levels so consequent relief in symptoms.

SSRIs especially Sertraline, Fluoxetine and SNRIs especially Venlafaxine are approved for use in anxiety and panic conditions. These drugs are relatively free of side effects and help in improvement of condition on long term basis.

Take melatonin agonists on daily basis for long as they will help in regularising the sleep rhythm.

Thanks, hope this helps you. If still have more unresolved queries you can ask again.

Wish you a good health.
