Suggest Treatment For Pain In Spermatic Cord

Question: I feel pretty worried like very worried. This is urgent but do please take your time. I am 18 and my spermatic cord just started hurting about an hour ago. It's pretty sharp up to my groin area. It only hurts mostly when I touch it and my testicle is a bit swollen and it hurts severely when I touch it. I turned my testicle around one time this week because it was uncomfortable. I really hope I don't have torosion because I have no way of getting to the ER. I have been fine all week until tonight. I was laying down on my side prior to this happening. Please help thank you in advance.

Hi XXXX! Good afternoon. I am Dr Shareef answering your query.

First of all do not get panicked for this as panicking would not solve your problem.

Torsion of the testis comes as one of the differential diagnosis, but as per the symptoms described by you, this could be one of the more common pathology in the area, i.e: epididymo orchitis with funiculitis. This is an inflammation of the testis caused by unnoticed repeated trivial trauma to the testis, which could commonly be caused by a cycling, while lifting heavy weight, while playing or due to forceful repeeted masturbation in the young, and due to forceful sexual acts in those who are sexually active and this could also be a sign of urinary tract infection (UTI). All these could occur more so in those where the testes are not properly supported by properly fitting under garments.

If If were your doctor, I would advise you for a urine routine/microscopic/culture and sensitivity test, and treat the infection if positive. In addition, I would advise you to go for a properly fitting undergarment for a sound testicular support, an anti inflammatory drug with a proton pump inhibitor, a broad spectrum antibiotic (after excluding a UTI) and to avoid any stressful work including masturbation which might cause repeated trauma to your testes.

However, the treatment described was for a inflammation/infection of the testes. In case the pain increases, or persists, and there is other constitutional symptoms, you would have to show yourself at least to your family physician , who might advise you for a Doppler ultrasound of both the testes to rule out a torsion. The management in this case would defer completely depending on the situation and findings.

Hello Dr. Shareef, Thank you for answering my question. I went to the gym Tuesday night and did some moderate lifting but nothing extremely heavy. Over the response time my testicle gradually starts to turn straight up and ride up but I keep fixing it due to I am scared. I am not cold or anything the left one is fine while the right one is still in slight pain. If I let go of the testicle it starts to turn up right and go up into my groin area. My left testicle hands lower on a normal basis but this turning of the right is all new to me since tonight.

Normally the left testis might hang a little lower than the right testis. This is anatomically normal and is not considered abnormal, and you need not worry about it.

Whenever there is a painful reflex to the testis, it will get elevated by its investing cremaster muscle which is an involuntary muscle (you do not have any voluntary control over it) and this is again a normal protective phenomena provided to us by God/Nature. Even in the winter you might experience similar elevation of both testes by the cremaster muscle for maintaining the normal temperature in the scrotum for the testes. So, you need not have to pull it down again and again which might rather cause more inflammation. As I explained you earlier, a good testicular support is very important part of the treatment program in your present condition, and so go for it first and then go for rest of the treatment.

Now I am hopeful that you would recover soon with the treatment, and would also avoid any stressful work till you are ok, and give your testes a good support all the time for future as well.
