Question: Hi,

I have a cervical spondylitis at C7 since 2010. I am 34 years old and my job is sedentary.

I have taken treatment from chiropractor for the entire 2012. I was feeling better during this treatment.

I had an accident while skiing. I fell on my head which resulted in chronic muscle spasm in neck for few months and then it got resolved on its own with the help of muscle relaxants.

From last one year, I am having gradually increasing pain, tingling, noticible weakness in neck, hands and overall muscular system. I feel tingling in hands and legs all most throughout the day if I observe.

I can't sit for more than one hour at the same place. I can't drive a car for more than 30 minutes. It is affecting me to perform basic functions and affecting my abilities to work. The condition is not improving even after frequent visits to Physio therapist.

I met neurologist. He asked me to take 4 injections of B12 as my B12 is 250. He advised brisk walk and visit after 4 months.

I am concerned about constantly reducing muscle strength, frequent cramps in hand fingers and legs, heavy neck muscle all the time. What should be my next course of action? Who should I visit? should I undergo any test?

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have read your detailed medical history and if you would have presented to me as an outpatient with this medical history,I would have told you to undergo 'MRI cervical spine with screening of whole spine' urgently because your cervical spondylitis is progressing and causing compressive cervical myelopathy.All your present symptoms in upper limbs and lower limbs are due to this compressive cervical myelopathy.I have a vast experience with these patients and there appears to be no doubt in your diagnosis.MRI will confirm the diagnosis.

Vitamin B12 level 250 is within the lower normal range.However,you should continue with injections.
In addition,please start-
Tab.pregabalin 75 mg once a day
Calcium and vitamin D supplementation.

However,the most important and disease modifying step is technically correct regular exercises.You are probably lacking on this front,which is most important for management.

Please meet an expert physiotherapist with good knowledge and reputation.Physiotherapy doesn't mean just traction and stretching but physiotherapist's true job is to teach exercises which target the specific joints and muscle groups and encourage the patient to keep doing at home regularly and strictly regularly.Also,learn from him life style modifications that how to sit and how to lie on bed?How much time to sit?
These things matter the most.Physiotherapy is to be done at home regularly and not like sessions at the physiotherapist clinic.
