Question: Dear sir,
My right testis is bit upper. Sometimes I feel mild pain too in right testis. Heavy excercise, or masturbation, and ejaculation causes it. It causes due to tight paints too. After taking rest it comes to normal position. Is it hernia? Is it severe?

Brief Answer:
Ultrasonography and color doppler.

Detailed Answer:
Hi.
Thanks for your query.
Read and understood that your right testicle appears to be higher (it is possible that the left one is at a lower level); mild pains sometimes caused by heavy exercises, masturbation and ejaculation, tight pants too. Comes to normal position after taking rest.

The cause of the upper level in your case looks to be due to body's protective mechanism that the muscles elevate it the moment body feels that there is a problem with it.

The pain can be enhanced due the activities as you have mentioned due to the inflammation in or around it.

Hence I would advise you the following:
First of all, get an ultrasonography and color doppler examination of the abdomen for genito-urinary tract and of the scrotum.
Secondly clinical evaluation, actual examination by the General Surgeon or an Urologist to ascertain the cause of the symptoms you have.
Thirdly, tests of blood, urine and semen analysis can help us to get an additional information.

All these things together can get us the diagnosis and a plan for further treatment.

This will also help us to see whether there is a hernia or not and what is the reason for all the problems you have.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications or if you feel there is a gap of communication.

Brief Answer:
get a proper diagnosis first

Detailed Answer:
Hi.
Thanks for your feeedback.
It looks that you are really worried about future.
Hence this is the right time to get a proper diagnosis so that your future is without any problems, without any sexual problems after marriage.

It is possible to comment on the issue of its cure by exercises or yoga only after proper diagnosis, as about the sexual function after marriage.

Night fall is not a problem, this is a natural phenomenon due to overflow of the prostatic secretions due to young age. Your present problems are not causing a night fall and will be stopped automatically once you get married.

I hope this answer helps you.

Sir,
I did colour Doppler of my right scortal yesterday & report is as follows-

Both testis & epididymis are normal in size & echo texture. Right testis measures 3.7*2.3 cm, left 3.3*2.3 cm. no focal lesion seen. On Doppler both testis show normal vascularity. No evidence of hydrocoele is seen. Evidence of dilated vascular tubular tortuous anechoic structures at the root of scrotum above testis on right side suggestive of varicocele. They show reversal of flow and increase in size(3.4mm) on valsalva maneuver.

Impression-
Right Gr I varicocele
Both testis are normal.
Please suggest me now about further treatment

Brief Answer:
3.4 mm varicicele.

Detailed Answer:
Thanks for your feedback.
As expected this is a symptomatic varicocele.
I would advise you the following:
Get semen analysis to see how much has this affected your sperm count and morphology.
Get as opinion of a XXXXXXX general surgeon or urologist.
Give me the reports so that we can discuss further.
