Question: I am a 80 year old male. I have been sailing for last 4 years have not even had a cold for the last 20 odd years I'm very fit for my age I work out 4-5 days a week. I don't smoke I drink about a bottle of wine a day. I left the boat after a domestic.2 months ago since then I have slowly gone down hill,
Last week I started to develope a lung infection coughing loss of appetite,I have lost 3.5 kg in weight have become very weak .please help me. Charley Barber.

Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. During periods of stress, immunity reduces and at present, you are having infection.
2. Due to infection, loss of appetite secondary to dehydration and reduced nutrition is common.
3. You need to continue with prescribed antibiotics. Along with it, good hydration ( atleast 5-6 liters of water/day) along with a protein rich diet is essential.
4. If you are having sputum production, nebulization or steam inhalation, twice daily will be helpful. If no sputum production and dry cough, you can take antihistamines like cetrizine or loratidine to reduce cough.
5. It will take around 3-4 days to recover. Hope you have no high grade fever, if present controlling it with paracetamol will be necessary.
6. At present, along with prescribed antibiotics, more fluids along with improved nutrition rich diet is essential. Mild activity is advisable to improve psychologically. Need to get regular sleep in night between 10-6 and mild rest is also necessary.
7. Wine can be continued after recovery and at present is no advisable, as risk of aspiration into lung is present.
