Suggest Treatment For Itching And Redness On Lower Part Of Leg

Question: Dear Dr,
I am suffering from itching in the lower part of my leg, it is lower part of knee mostly near the leg bone. I observed some red and black spots. I feel more itching near the redness area & skin is also shining on those area.
I observe more itching during summer but other time also I feel the same bt not much. so it is not time specific.
I have some image to upload but not getting any option here, pls suggest.

Hi,

Welcome and thanks for posting your query to the forum.

I can understand your concern for the itching in the lower part of legs with spots.

After analyzing the pictures and correlating with the history I can make out the best possibility of "Contact dermatitis" or in simple words allergy to contact of some allergic material on legs.

To explain you elaborately about the condition - Due to exposure to any allergic material such as any tight socks or more sweating or heat and humidity with the areas can cause the problem. So there can be initial redness and itching with mild itching sensation. As we scratch it, then can get aggravated.

This is a quite simple problem which we see in our day to day practice and persist for only the time till the aggravating factor is present. So the most important part in your management is avoidance of the aggravating factor enumerated all above. In your case it appears to be the tight rubber of the socks compressing the calf area.

For the betterment kindly follow the suggestions below:

1. Avoid tight socks and rather wear socks of lose nature upto ankles. Avoid hot – humid climate.

2. Wear cotton clothes and avoid synthetic clothes.

3. Stay in cool environment (preferably AC) and avoid sweating.

4. Wash your legs two times or even more which will soothe your skin and prevent the aggravation of the problem

5. Do not Pinch pop or squeeze or rub any area.

6. Do cold compression over the affected area. Take a wet towel soaked in cold water and apply it for 5-10 minutes for 3-4 times a day.

7. Do not stand for long times as long standing causes pooling of blood and then more urge to itch.

For the part of treatment I suggest following measures to my patients:

1. Apply over the counter available calamine lotion on the affected area in day and afternoon time. Apply it in very thin layer.

2. Apply plane hydrocortisone cream in a thin layer in morning and at night time. Apply in a very thin layer so that it should not be visible to naked eye. You can apply it for 7 days and then stop it after that.

3. If there is any irritation or itching is present then can take tablet “Loratadine” or "Deslotatadine" tablet by mouth up to three to four times also if the itching is more.

Cream not to be used beyond 7 days.

Hello,

Welcome back and thanks for your swift follow up.

In my patients I suggest to apply hydrocortisone cream (Cutisoft cream -Ipca pharma) over the affected areas. It is to be applied very thinly over the red areas only. It is not to be used beyond 7 days. Rest, other of the suggestions are more important to prevent the further formation of such kind of spots.
