Question: I'm taking Cilacar 10 and Tellista 80 in the morning. These two drugs were replaced by doctor on 1st May,2014. Before these I was using Losar A 1 Tab & Nebicard 5 1 Tab in the morning and Losar A 0.5 Tab & Nedicard 2.5 Tab in the evening. But due to pedal edema Doctor has stopped amlodipin and other also. He the prescribed what I'm using now.

But the problem is that my High Blood Pressure still continues to be 154/94 or very close to it. What should I do now.

Dear XXXX

Greetings from health care magic!!! I am Dr Thakker and shall be answering your question. I can understand your anxiety with regards to high BP. I will try my best to help you with it.

I have been through all your attached reports.

You have 2 proven problems: Hypertension and Dyslipidemia.

For hypertension: Your doctor has done the right substitution with cilacar. This is a new calcium channel blocker which does not cause leg swelling. Since cilacar and telmisartan are not enough, we can step up cilacar to 10 mg twice a day and continue tellista 80 as before. This is XXXXXXX dose of cilacar which we can give you. (20mg) . We then recheck your BP after 4 days. If it is still more than 140/90 we can add a new drug which is alpha blocker or a diuretic drug.

For Dyslipidemia: You have low levels of good cholesterol HDL. I would recommend you to exercise, walk 30 mins daily for 5 days and do diet modification. We can add a drug which increases HDL. I cannot prescribe medication names as per rules of the website. Also the LDL level is 110. I would suggest a target of 100 for you.

With regards to sugar, I wish to know your fasting and post lunch sugar levels. I also wish to know your uric acid levels.

I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.

Yours sincerely,
Dr Thakker.

Dear Sir,

Thank you very much for your cordial reply.

I'll increase the dose to Cilacar 20 as you have advised.

I'm to inform you also that I do not do any exercise at all. I also have to do sedentary works most of the time. I am also concerned with the HDL and LDL level. I am going to purchase a motorized treadmill as morning walk is not possible for me in this area. I also am taking Almond and Walnut as I've heard these increase the HDL level.

Please suggest me whether all these activities are correct or not.

With regards.

Yours sincerely,
XXXX

Brief Answer:
essential fatty acids, almonds and treadmill

Detailed Answer:
Dear XXXX

Buying a motorised treadmill is an excellent idea to boost up your metabolism and modify your lipid profile favourably. I would suggest 30 mins daily in the morning for 5 times per week.

Another suggestion is you can take essential fatty acid supplements available in the market. They will help you as well.

You can have half an ounce of plain dry almonds everyday.

I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.

Yours sincerely,
Dr Thakker.
