Question: Two weeks ago I had an episode of vertigo. Never one before. Woke up in the morning very dizzy. Went to Out patients four days later - Dr. described vertigo. Take some anti nausea meds at bedtime. It worked - most of vertigo gone now about three days…but there is hearing loss, ringing and pressure the last couple of days in my right ear. Very annoying. Can't get to My Dr. for a week…any suggestions

Brief Answer:
Sounds like BPPV, but these new symptoms need re-evaluation.

Detailed Answer:
Hi,

Thank you for your query.

1. Your history suggests that you had an episode of classic BPPV (Benign Paroxysmal Positional Vertigo).

2. However, these symptoms of hearing loss, tinnitus (ringing) and fullness (ear pressure) are usually seen in Meniere's Disease.

3. There is no way without a hearing test (PTA-Pure Tone Audiogram), Tympanogram (I/A), eardrum examination and vertigo tests that this can be sorted out.

4. A re-evaluation is necessary as these are not the symptoms associated with BPPV.

5. The possibility of side effect of medications (drug-induced), blood pressure related and a different process has to be kept in mind. In your case a recurrence of herpes zoster oticus should be ruled out.

6. A salt restricted diet, avoiding over-hydration and informing your physician should be the immediate step forward.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.

The hearing is clearing up, as it did in XXXXXXX when the same ear was 'plugged'. I took antihistimine as directed by Dr at that time

Brief Answer:
Fluctuating hearing loss suggests Meniere's Dusease or OME / ETD.

Detailed Answer:
Hi,

Thank you for writing back.

1. If your hearing has been clearing up at times, it is more in favor of Meniere's Disease or Otitis Media with Effusion (OME) due to Eustachian Tube Dysfunction (ETD).

2. A hearing test (PTA-Pure Tone Audiogram), Tympanogram (Impedance Audiogram), eardrum examination and vertigo tests are really important now for further clarity regarding your condition.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
