Suggest Treatment For Frequent Urination With Erectile Dysfunction
Question: Good Morning Doctor,
I am XXXXXXX 19 years old male. There is a discomfort and painful feeling(urge) in the penis for the whole day. This is causing me a stressful feeling and I am not able to concentrate in any thing except that thing. Its very difficult even to describe the feeling. It feels as if the semen is in the tip of the penis and even a slight press by thighs, it spills out. I feel as if it is(semen) is in pipe. It never happened and its a very painful experience.
In December I went to the urologist for frequent urination problem. I just told him that I dont get an erection in mornings. He suggested me some tests and medicines which I have attached for you.
But after taking the suggested medicines only for 3 days [Modula 5mg and Freecharje] I had slight erection all the day which made it very difficult for me and a constant urge to masturbate. My frequent urination problem didnt solve so I consulted a Homeopathic Doctor who gave me few medicines along with a syrup [BAKUNIL] which I took for a week and my frequent urination stopped. But, then after I started facing the problem which I wrote in para 1. I have slight erection all the day and feel as if the semen is in the pipe but even after I masturbate I still get that discomfort which makes it very hard for me to concentrate on any thing.

In your reports, I found the Yeast cells+ in urine. Other tests were normal. This yeast may be causing frequent urination. It is a very common finding nowadays and its part of normal flora. Yeast cells are usually a side effect of antibiotic use and may last for quite a long time. In most of my patients, it resolves after lifestyle measures like healthy nutritious food, hygienic food and cleaner drinking water from RO system regular exercise including yoga. If you try these I am sure your current symptoms will go down. Lots of fluid intake can help wash out any semen in the urethra. Since there is no bacterial infection antibiotics are not needed.
Modula is tadalafil (similar to viagra). That is why erection all day. I would not suggest it unnecessarily. It is meant to be taken when needed prior to sexual intercourse if there is erectile dysfunction. Since you have stated that you masturbate (which is normal finding), I don't think it should be taken.
If the symptoms persist after 2 weeks of trying lifestyle measures, I would suggest that you see a surgeon. He may do tests like VCUG, etc to confirm cause.

Thank you doctor for your answer. Yesterday I had a chat with a doctor who prescribed me Tab.Fluconazole 150mg and Citralka 10ml syrup. Shall I recommend my son to take it ?
And doctor, a constant urge to masturbate and a feeling as if the semen is going to spill out is normal ? Because he never had these things before. Seeing my son crying out of pain makes me extremely sad. It is very difficult for him to concentrate in his studies.

I am glad you came back with this query about fluconazole. It is an anti-fungal and I will NOT suggest it to your son at this young age. It can have side effects. Your son is already experiencing side effects of tadalafil. Things may get worse.
The yeast infection slightly cleared up with homeopathy but is back again. In my experience medicines clear up yeast infection temporarily and it comes back.

After i went home, I had a talk with my son. He said that his penis is in flaccid condition. He is not feeling erected. He took 1 dose of fluconazole yesterday night and Citralka syrup 1 spoon with water yesterday night and today morning. He is feeling 10% improvement. Doctor you suggested to go to a urologist, But doctor as I already have a very bitter experience with one urologist I am scared of going to urologist again. If you know any good urologist in west XXXXXXX you can suggest me. Also please tell me if he can take the Citralka syrup and not taking the fluconzole. Pl suggest me.

Since your son is complaining of pain, he has to be evaluated at the earliest. It cannot be ignored any further. The evaluation HAS to consist of some imaging like ultrasound/ doppler ultrasound of penis. Whenever you go to doctor, please ask for definitive diagnosis which can happen only after testing. Do not take any medicine unnecessarily, it may spoil your son's health. We need a diagnosis so that appropriate treatment can be started. For this you have to visit a surgeon who will listen to you patiently. Keep visiting one urologist after another until you find a urologist who understands you.
