Q: I have been suddenly suffering from frequent urination and consistent mild to sometimes high pain and also burning sensation thorought the day. Abdominal and pelvis CT SCAN without contrast was done just few days before the issus started as initially i was having severe abdominal pain and constipation and anal fissures and it came out completely normal. There is blood in urine which was 10-12 cells/hpf 4 days before and today smear test stated it reduced to 1-2 cells/hpf. Protein was also detected 8 days before with value as 47 but today it came to 12. I have also done XXXXXXX ANA,GBM and Urine dysmorphic test and all is normal. Doctor did a finger test for my prostate and it pained like hell in my pelvis area during the examination so he advised antibiotics. But when i got and ultrasound done and urine culture, it came out to be clear. The pain and burning in penile area is still there and i am just wondering do i really have prostatitis coz ultrasound was normal. Also, should i trust finger test better and start taking antibiotics?

As an Urologist, i don't think it's a typical urinary infection(UTI ).
There're certain details and tests needed to arrive at a provisional diagnosis.
Your age and previous medication information is needed.
The tests to be done are :
1.urine -AFB,for 3 consecutive days of morning urine.
2.blood routine,sugar,creatinine and PSA.
3. TRUS (trans rectal ultrasound scan) of prostate.
With these tests reports and details as mentioned, i can give you an expert opinion.

Thanks doctor. I have got few tests already done and need clarification for few as mentioned below:
1.urine -AFB,for 3 consecutive days of morning urine.
-- >> does this mean the sample should be the first urine passed?
2.blood routine,sugar,creatinine and PSA.
-- >> Blood routine,sugar is all fine as per the latest test. Creatinine was 1.4 the first day after I was admitted for abdominal pain and reduced to 1.3 the very next day. Final test just 4 days before shows the value as 1.
3. TRUS (trans rectal ultrasound scan) of prostate.
-- >> does this mean normal Ultrasound can't detect prostatitis. I will follow up with the doctor.

Can you please advise if I should start taking antibiotics from today only before tests are available? Does the finger exam confirm to some extent that I might have prostate issues?

1. First morning urine sample.
2. creatinine is normal. Kidneys are functioning normally.
3. Usual antibiotic is recommended.
A rectal examination is essential for prostate gland evaluation.
TRUS, is most accurate for prostate gland evaluation, compared to abdominal ultrasound scan.
