Suggest Treatment For Fever, Severe Cold, Cough And Upset Stomach

Question: Hello Doctor,
I have twin daughters and they are 2.5 yrs old. A month ago one of them developed severe stomach infection
and was constantly puking, we took her to a doctor and he gave us medication from which the stomach infection
was cured. Since then both the girls have been getting fever repeatedly. Every week they will have 3-5 episodes of fever
which will come down as soon as we give them Calpol 250 syrup (fever ranges between 100-103). They also have severe cold and cough since a week.
We took them to a doctor last friday and the doctor said it could be a viral infection and since they have no trouble with them passing stools and no stomach upset (they are eating well) then we should just administer the fever with Calpol and
let them slowly recover. He has also given some medicine for cough if it becomes severe.

We started putting them to day care recently and the doctor says it could be because of them getting exposed to a new environment.

Our worry is that they are getting high fever frequently (since last friday at least 5 episodes each, especially in the night) and we keep giving them cold sponge and then calpol 250 mg. Since they are taking too much medication and their fever is repeating we are getting worried that there might be something else that we are overlooking.
Can you please advice how to go about this? how long should we wait before we take them back to the doctor?

I have gone through the query and do understand your concern.
In viral infection fever usually starts declining after 3-4 days. In your case fever is there with cough and cold,earlier fever was associated with stomach upset. So its really difficult to comment whether the current illness is a continuation of previous one or a recent new infection(as two are presented with different symptoms).
Still as fever is there for more than four days and severity is same as earlier I would better like to go for few investigations like complete blood count, C reactive protein and urinary routine examination to rule out any bacterial infection(clinical picture is unusual for viral infection).
If investigations are suggestive of bacterial infection antibiotic needs to be started and if it comes normal then this can be due to unusual presentation of viral infection. Meanwhile continue to give paracetamol and do sponging. Give plenty of fluids to avoid dehydration.
