Suggest Treatment For Explosive Diarrhea In An Infant

Question: My 17 month old grand daughter has had diarrhea for about a week It is very runny and the diaper will not absorb it all when she has a BM. It runs over the side and up her back. Her appetite isn't good and sometimes she will drink and sometimes she won't. We have been giving her pedialiyte when she will drink it and other liquids other than milk that she will drink. She has vomited a couple of times over this period.
She doesn't have a fever. Sometimes she does feel a little warm but nothing on thermometer. She is very irritable--not like her. A call to the doctors office suggest Culturelle. She has been on that for 3 days. Not helped. Looks as though she has lost some weight. can you help She had two wet diapers yesterday in a 9 hour period.

I have gone carefully through your query and understand your concerns. It seems that your grandchild has an episode of gastroenteritis or food poisoning. It could be due to contaminated food or improper hand hygiene.

In case she has further episodes of vomiting you need to give her some antiemetic like ondansetron as and when needed, especially before feeds (15 min-30 min prior to feeds). The mainstay of treatment is breast milk and ORS (oral rehydration solution)/pedialyte. Unless the lost salts and fluids are replenished the child would become dehydrated. The urine output needs to be monitored. If the frequency comes down it indicates dehydration and calls for additional supplementation of fluids. But if she becomes lethargic you would need to take the child to a hospital without delay. Otherwise you would need to patiently continue pedialyte and gradually in a few days the stool frequency will come down.

You need to be patient. The vomiting and loose stools may persist for a couple of days before it resolves.

Culturelle contains probiotics which has a general beneficial action. Also give curd or yogurt since it contains plenty of beneficial bacteria and should help. Give zinc supplements like Zinconia Syr 5 ml once daily for the next 10 days. You need not worry. It should gradually return back to normal.
