Brief Answer:
Some investigation needed for diagnosis

Detailed Answer:
Hello,

Thanks for writing to us.

I can understand your concern.

Your back pain and sudden onset of erectile dysfunction required some investigations.

You need to go for blood pressure check up, serum testosterone level, penile doppler and lumbrosacral spine MRI for detail evaluation of your problem You can consult any psychiatrist, Orthopedic or NeuroPhysician for these investigations and examination. Or you can go through these investiagtions on your own and revert back with reports.

Dear sir,
I have gone through some tests as suggested which i have attached with mail.

Hb : 14.6
RBS : 75
Serum TSH : 1.16 miu / ml
Serum T level : 273.31 ng/dl.
( tested at 11.30 am )

Now pls suggest me whether should i go for penile doppler and lumbar mri test .
I m feeling better in my lower back pain after doing some exercise and yoga and pain reduces as day progresses.

Now what should i do to treat the ED.

Thanking you

Brief Answer:
Reports are normal

Detailed Answer:
Hello Mr XXXXXXX

Thanks for follow up.

Your reports which have been done are normal. This rules out the hormonal problem as cause of ED. But still lack of morning erection, loss of muscle mass and back pain suggest some chances of some physical cause otherwise your problem is more psychological.

Two options from this point now

1. Assuming your problem is psychological: you can start a supplement - vitamin called tetrafol plus (methyfolate+ pyridoxal phosphate) two times after meal. You can take initially for 15 days. If you feel improvement, complete the course of 2 months. You can get this medication with doctor prescription.
Also go for sexual counseling with your partner. Master & XXXXXXX technique (6-7 sessions required) is quite helpful in such cases.
If you can masturbate normally and get good erection and orgasm, your problem is 100% psychological.
Follow above measures and if no improvement, go for remaining investigations.

2.GO for Penile Doppler and MRI to rule out other physical causes and move forward according to reports. If both reports are normal, what I have mentioned in 1st option is needed. If report shows abnormality, specific intervention needed according to problem.

Hope I have answered your query, I will be glad to tell more if need.
Regards,
Dr.Chintan Solanki.
