Suggest Treatment For Elevated SGOT And SGPT Levels

default
Posted on Tue, 14 Jun 2016
Question: Appointment with the VA Hematologist on 4-12 resulted in a dramatically elevated serum iron to the point where I'm told my liver is in imminent danger. I believe I exasperated this situation by over doing the brandy I drank the night before the blood test. Generally, the numbers that raised concern are lower to normal in the summer. The only drinking I do in the summer is light beer, about (6) 12 oz cans per week. Here are the numbers and dates on the oncology report that were highlighted by the VA Doctor:
Plasma
SGOT: 5/11/2015 31, 1/22/2016 51. 4/7/2016 80
SGPT 5/11/2015 38 1/22/2016 51 4/7/2016 111
T BILI 5/11/2015 1.1 1/22/2016 1.4 4/7/2016 1.4
Blood Profile
HGB 5/11/2015 12.3 1/22/2016 10.3 4/7/2016 11.0

Brief Answer:
liver functions have indeed worsened

Detailed Answer:
Hi
Thanks for followup.

Liver functions are worsening and definitely alcohol is one of the culprits. You have said iron dramatically increased but report has not been provided.

I would suggest complete abstinence, getting a full workup for liver disease including ultrasound and fibroscan. Treatment may include liver protectants like ursodeoxycholic acid, antioxidants and iron chelation as appropriate.

Hope this helps.
I will be available for followup.
Regards

Misidentified serum iron as having increased. I don't recall discussing that with the Hematologist specifically. Our discussion was centered on the liver condition. Am scheduled for another blood test at the end of the month and, frankly, do expect notable improvement in the pertinent numbers. Have drastically reduced alcohol consumption to two cans of light beer on Monday and Thursday (golf days). Will not be using my winter brandy ration during the summer in any case. Will report back after next blood work and also request my VA Primary Care to schedule the tests and treatment protocols you recommend. Thanks.

Brief Answer:
ok this plan seems fine

Detailed Answer:
Yes probably iron was not repeated this time.

The current plan seems fine. Abstinence is most important.

Let me know what happens.
