Suggest Treatment For Drowsiness Even After Having Proper Sleep At Night

Question: I have been having very sleepy times here at office.
Earlier I used to take that I am sleeping less in night time(6 hours) so I am having this situation, but last night I slept for 8 hours, did some yoga and 8 kms cycling to office, I should fee; active, but I am getting sleepy all together, which is affecting my work also. it has been since I have joined the office, i.e. 3 years, in college after lunch lectures were sleepy , but I never took it seriously thinking that I had less sleep in night, but today all failed with really bad morning which I tried to make very much fresh.
Please suggest.

Hello,

You are right that 6hrs sleep might be less for some one amd can make you dull and sleepy in day time. But same can be due to sudden starting of cycling and yoga can also make you tired and thereby leading to sleepiness!

Can you try doing just one thing at a time in the starting - like- sleep good 7-8 hrs and do small shift of yoga and goto office. Then later after a week or two add cycling too..then we can be more clear about the situation.

Secondly- how do you sleep at night ? What is the quality of your sleep? Is there any difficulty in going to sleep or do you wake up in between?

How do you dream in sleep..? Or not?
If there is no sych problem - then we try to rule out sadness or anxiety behind all this.

There are medicines to help this excessive sleepiness in daytime so dont worry at all.
Just try to asnwer the above questions in detail inorder to diagnose the condition correctly.

Hello Ma'am,
thank you for your response!
I have been cycling since 1.5 years, to office, earlier it was 4 kms one way and now it is 8 kms. I love it, there is no sudden shift. in case of yoga yes, I used to do lot of XXXXXXX namaskars everyday, then all stopped because of cycling and the project I am working on, "Cycling Cities" to promote cycle as utility vehicle, for which I have calls with my spread up team across cities, so there are late night conference calls, discussions, etc.
About the quality of sleep,
I don't wake in between much , just once for toilet, and there is no difficulty in going to sleep as I am so tired after office and this project work that I sleep effortlessly, and yes I do dream, sometimes I remember, sometimes I don't, but most of the times I have dreams.
About sadness, I don't have much, I am excited and active most of the times, sometimes stress or anxiety is there which also leads to stressed situation and breathlessness also.
Let me know what can be the possible reasons.

Hello,

So, according to you the cycling and yoga both are regular in your life and there is nothing new about them and the problem of daytime lethargy is also an old one .

There is no sadness as per you but there is on and off anxiety. Technically anxiety due to some major issue is taken to be normal, but it will not lead to breathlessness to most of the people.

Can you please tell me-
1. if you get attack of breathlessness whenever you have stress and anxiety of significant degree?

2. Is this breathlessness also associated with feeling of doom and fear of death or heart attack like thing and sweating?
{This can be then a panic attack like symptom}.

3. Also try to recall how manytimes you have such attacks in a month?

4. And the duration till the attack lasts.

If the symptom crieterias of panic attack are fulfilled then you will need a different drug and if not then modafinil would be of help.

Please get your thyroid, vit D and Vit B12 level along with complete blood counts. Treat if any if them is not in normal limits.

My answers are below:

1. In april I had maximum attacks of breathlessness and I consulted doctors here and in person also, and it came out that I have just allergy and nothing else and I was going for cycling trip in Himalyas and I was worried if breathing problem will create any troubles there, but to my XXXXXXX in May start, during the trip, I had no such problems and I came back I was completely fine in whole May, XXXXXXX and July also, just some issues happened last week of breathlessness and that I realized maybe because of haste and anxiety.
2. no such feeling of doom and fear of death or heart attack like thing and sweating

3. that time in april, I had so many of such breathlessness cases, like couple of them in a day, but I cant put them in category of attacks, as I used to feel so exertion that I had to breath long from mouth instead of nose.
4. And cant say duration as for couple of minutes I used to breathe heavily from mouth, mostly while eating it used to happen.

And to my knowledge and last reports a year back, the above levels were blood and other fine and I have a feeling that I eat lot of sweet and maybe because of that it happens, my yesterday's breakfast was sweet sevaiyyan kheer which might have lead to this and in lunch also it was sweet in canteen, so might have been the case and I have sweet tooth, Hypoglycemia can be the situation. Can you throw some light on this?
Today I am doing fine and not leaning on chair and no sweet product taken since morning.

After reading your version even I dont think that it is anxiety.

The sweet excess would lead to hypergylcemia and not hypo...and it would then lead to feeling of lethargy and dizzyness . This happens coz the blood is pooled in abdominal organs for digestion of food..and therefore it goes less in head- leading to dullness and sleepiness..
.
Breathlessness can be found in low blood pressure too....so, for once or twice you may get it checked during an attack.

I think if all tests were ok then you need not repeat them and you can therefore start modafinil 50mg half tablet after breakfast. After a week raise it to one tablet.

Try not to take it in evening hours as that might disturb your sleep.

Continue to do cycling ..its good for you and also for the environment!!

Hello Mam,
I was not able to ask follow up question because of absence over weekend.
I would like to know that the drug which you have advised is for which ailment?
As I don't prefer taking medicines, here I would like to know the root cause of the situation which is happening to me, is it food getting converted into sugar and affecting brain simply or something else?
and would like to know if there are natural remedies for curing the same.

It is always good if we try natural measures first and if nothing works only then move to medicines.

In your case, as I mentioned before, there can be a big possibility of sugar overloading giving dullness. You also mentioned that when you cut the intake you have more freshness !

The problem with you is known as chronic fatigue syndrome. Even if all features are not found but depending upon the intensity of main symptom diagnosis can be made.

I think you must religiously try cutting down on your sugar or food intake espically in morning and early afternoon time. I think if that solves the problem even upto 50-70% ,it should be sufficient.
And if that completely resolves the leathrgy and sleepiness then you need not worry at all...

already you have an active lifestyle which is a basis of a healthy mind..i think you will be significantly benefited with this approach!!

Try this diet control for atleast 10-15 days and see how it works.

--Root cause can here be decided only when you follow that dietary restiction. Psychiatric diagnosis is a diagnosis of exclusion. When all physical causes are ruled out and nothing else can be held responsible for the particular symptom(s), then we diagnose them to be psychological. So, we need to rule out sugar intake causing dullness in you before we can be sure.

P.S. this drug (modafinil) is an activating molecule which helps in removing lethargy and making one more alert.

yes I will take care of the food intake or will try to take it in small quantities than too much at a time and will se the effect of the same over 2 weeks.
And hope it doesn't reach the condition of medicines.
