Question: I have been having digestive problem since last 1 year. I feel I do not flush out completely. It has affected my life a lot. I have tried Miralax,MOM,Triphala,Kayamchurna,Salt water flush,Colon cleansers. I have a bowel movement everyday, but the thing is I do not have bowel movement I used to have a year before.I feel its always incomplete and am not able to flush out. Its been almost a year I feel uncomfortable.Loss of appetite, weakness.Even if I do not eat anything I do not feel much hungry. Is there any medicine with which I can completely flush out. I eat healthy and am a vegetarian too. My doctor told me to take amitiza but I cannot take that as it contains gelatin.

Hi.
Thank you fro posting your query at HCM.

I understood your concern and would try to reply in the best of my capacity.

Constipation definition varies from person-to-person. Stools may be too hard or too small, difficult to pass, or infrequent (less than three times per week). Some people with constipation may feel a sense that the bowels are not empty ( that maybe in your case).

Firstly, since the problem is experienced for almost a year, i would recommend you to visit a general surgeon/ gastroenterologist to perform flexible sigmoidoscopy OR colonoscopy to rule out any lesion in the intestine.

Furthermore, if you have observed blood on the toilet paper or loosing weight or family history of colon disease, then Fecal test for occult blood is and colonoscopy is highly recommended.

Meanwhile, diet modification is advised as follow:

- discontinue any medication for constipation
- thrice daily , take two tea spoon (or one big spoon) isphagula husk with 200 to 300 mL water
- increase warm water intake as much as possible
- eat small meals (rich in fiber)
- eat plentiful "fresh oranges" and "pear "
- avoid COLD milk and and AVOID junk food(NO burger , pizza) altogether
- mix few drops of "lemon juice" in warm water. Drink on empty stomach, early in the morning, to
aid in bowel movement
- avoid beverages / alcohol/ soft drinks
- do some exercise (push-ups etc) early in the morning
- avoid carbonated drinks
- take 2-3 spoon edible virgin olive oil in the morning
Any further questions are most welcome.
Hope to answer your concern.

Wish you good and sound health.

Regards,
Dr Tayyab Malik

Hello,
I have never had blood in my stools,no weight loss or any colon disease in my family.I have taken a lot of laxatives since one year.I think that may be the case for this condition. Yes I have consulted a gastroenterologist and he told to take Miralax for 10 days and come back again.I will be visiting him on coming Monday. I go to toilet daily and have bowel movement but it is always incomplete. Is it possible to have loss of electrolytes and am having little difficulty in concentrating on things as I feel there is something wrong in my abdomen esp. since last 3 months. Also I do not pass much gas. I have taken Isapghol for 5-6 days, but it was slightly painful and used to feel bloated with it and it used to be the same feeling of not having flushed out.I have started drinking coconut water daily to restore electrolytes if there is loss of it.

Hi.
Thanks for getting back to me.

I will address your queries one by one.

If no blood ever observed on toilet paper or in stool , then most likely there would be no chronic problem. However, one needs to undergo colonoscopy to confirm.

Secondly, you may discontinue laxatives and NO need to take isphagula husk too. However, your advised :
1. to ingest "pear and oranges- daily
2. mix lemon juice in warm water and drink on empty stomach daily for at least a week or 10 days.
3. ingest olive oil ( 2 to 3 spoon)
4. small meals (rich in fiber)

Lastly, loss of electrolytes is UNLIKELY in your case as you did not suffer from diarrhea and/or vomiting. serum electrolytes assessment can confirm this.

Hope to answer your concerns.
I will be glad to clear any further doubts.

Regards,
Dr Tayyab Malik

Hello Doctor,
I. have been having slight stomach pain in my stomach always and it feels very uncomfortable. It has been worse since last 2-3 months ,it is like my life has come to a halt and I have tried everything.2 months ago laxatives had stopped working, and I never felt hungry at that time, then I went to doctor at that time, he took my X ray and said that there is waste accumulated on 2 sides of my large intestine and told me to take amitiza for 10 days but I could not take it as I am vegetarian and it had gelatin in it.T then he told to take high doses of Psyllium husk and I took it for a week, but it was a little painful and I used to get bloated.Even now I feel that there is something stuck in there, and no matter what I take It doesn't clear it up,I want to get normal again. Now I have been on Miralax since I came to a gastroenterologist and I have my next visit with him on coming Monday. I don't remember a day I didn't take any kind of laxative in 2 months.Mostly I used to take Tripahala ,but I tried Miralax,Milk of Magnesia,Psyllium, colon cleansers, salt water flush. I want to have a normal bowel movement without all these laxatives. I have been having a bad digestive system since last 2 years, since I have come to United States because of various factors, but this time it has become worse.

HI DEAR.
i understand your concerns.
i hope to answer your concerns in the best of my capacity.

first and foremost, you should NOT stress at all. It will aid in resolution of your current symptoms. Be positive and be confident that steps you would take will help you efficiently evacuate your large bowel.

now, avoid taking any laxatives and medications.
have meals - rich in fiber. Do not eat mouth-full. Avoid rice for few days.
Drink warm water , atleast 8 to 10 glasses a day.

as mentioned previously, eat plentiful pear and oranges.
try to go for a morning walk early in the morning , ( walk atleast 20 to 30 minutes). Then, mix lemon juice in "warm water" and drink on "empty stomach" daily for at least a week or 10 days. This would most likely help in the effective bowel movement.

and last but not least, colonoscopic examination by a gastroenterologist may help visualize the entire large bowel to rule out any organic cause.

hope to answer your concern.
wish you good and sound health.

Dr Tayyab Malik
