Chronic back pain is usually caused by spine problems including spine disc herniation. Spine disc problem can give neck pain (if the problem is in cervical spine), middle back (thoracic spine) and lower back pain which can radiate to the legs (lumbal spine).
First of all you should get proper diagnose. To do that, spine MRI should be done. This test will tell how advanced are the changes. Depending of this test results you may have surgical treatment or physical therapy and medicines (pain killers mostly).
I suggest you to consult orthopedist with this issue.
