Question: About 5 weeks ago I had what I thought was an ordinary bladder infection. For 2 weeks I tried treating it with more water and less coffee. On the 26th of Sept I contacted a dr (I was out of state) because of severe burning on urination and was given 7 days each of Sulphameth/trimethoprim 800/160 and 100 mg of Phenazopyridine. 4 days later I had 102+ fever and extreme nausea and headache lasting 4 days. Then I flew home, called my dr and was given 1 week of Levaquin 250 and I had a Ua with greater than 100,000 E. Coli (among other info) I have often in my life had a problem with reflux (and subsequent surgery 50 years ago) and I think this has caused my kidney to be involved with the infection. I saw the dr. Monday of this week he seems very unconcerned, and was given Macrobid 100mg 2xday for 7 days (I'm on the 5th day) and told to come back if it wasn't better in a week, but still having Kidney pain, low grade fevers (that don't last too long) and fatigue The burning sensation is finally gone but urgency is not.

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

After going through your clinical history and query, my opinion is as follows:

1. You have been on multiple antibiotics and the last Macrobid, appears to be working. Continue the therapy.

2. After another 3 days, get a repeat urine microscopy done. If positive for leukocytes, then still infection is present. If you still have the sensation, you might be having antibiotic resistant bacterial infection. If after another 2-3 days, if you still get back the burning sensation and urgency, you can get a culture and sensitivity of urine sample. Based on that, a sensitive antibiotic to bacteria can be started.

3. Yes, reflux at young age increases risk of infection during childhood. However, if it were to be secondary to it, you would have been suffering all along these 50 years. You also had a surgery for it, so it is not responsible for your present UTI. You must have got a resistant infection and it will take a longer time.

4. UTI at present could be due to dehydration mostly. Take plenty of water. Avoid public unclean toilets. Females are at high risk of UTI due to short urethra. Maintain hygiene.

5. Do get treated at the earliest to avoid any complication. Take good amount of fluids and maintain a active life.

The disappearing of burning pain is one of the symptom of improvement. It must be slowly responding. Fever is low grade, so possibly reducing or weakening infection.

However, if still not controlled after another two or three days, get a urine culture and sensitivity done. Based on sensitivity, appropriate antibiotics can be started.

Importantly, take good amount of fluids. Take protein rich diet to improve immune system.

Get treated sooner to avoid infection complications like renal disease or systemic infection - spreading to other parts of the body through blood.
