After reading short history and seeing the photo you posted, I deeply feel that most probably you may be having scrotal dermatitis. There may be itchy,dry,mild scaly, pigmented patches on the scrotum. It may be exacerbated by soap,perspiration and excessive bathing. You consult dermatologist personally to have firm diagnosis. However, I would like to suggest as below...

- antihistaminics like levocetirizine to relieve discomfort and itching
- steroid like methylprednesolone in tappering dose
- local application of mild steroid like mometasone cream
- Sitz bath..to sit in water tub containing few drops of dettol for five min daily
- avoid stress and tension ..that might exacerbate the disease

Have patience for the good result. Keep good hygiene and have healthy diet.
