Suggest Treatment For Balanitis

Question: Hi, I have been diagnosed with zoons balinitus. I was prescribed Hydrocortisone 1% m/m ointment. 2 weeks twice a day, 1 week once a day & 1 week every second day. It is situated on my foreskin. I get relief for about one week & it seems to start again. I don't have any pain bit it is an eyesore & as a result my partner refuses to have sex. What else can I do ? Will an application of manuka help? Thank you in anticipation, XXXX.

I am dr.kakkar(dermatologist and venereologist).

I would suggest you to use a moderately potent topical steroid e.g mometasone furoate 0.1% ointment rather than topical hydrocortisone which is a very mild steroid and unlikely to be very effective Or else you may use topical tacrolimus 0.03% ointment which is also an effective treatment option for Zoon's and is also non steroidal.

How many times a day should I use these new ointments & for how many days? Will this clear it up altogether?

Hi.

Use one of these, twice daily for 3-4 weeks, initially.
Usually it does clear on medical treatment, however, it may require repeated or intermittent applications. But in those in whom it does'nt clear Or those in whom it recurs, circumcision is the definitive treatment.
Manuka honey is not an approved treatment for Zoon's balanitis and therefore I would advice you to avoid it and rather use recommended treatment options.

Thank you for your reply. After the 3 to 4 week application & the condition had not cleared up can I start another application immediately or must I leave a rest period before I start a repeated application?
How long should an intermittent application last?

Detailed Answer:
Hi.

After 3- 4 weeks of a moderately potent steroid, it should improve and then you may discontinue with the steroid cream.
However, since the condition may return back after some time after you have stopped using steroid cream which would necessitate intermittent applications of the steroid cream (for 1-2 weeks), as and when required Or switching to a non steroidal topical e.g tacrolimus ointment.
Steroid creams cannot be used regularly. You may either use them intermittently (for 1-2 weeks) if the condition recurs infrequently or else you may switch to a non steroidal topical application e.g tacrolimus if the recurrences are frequent.
Tacrolimus ointment is itself an effective treatment option for Zoon's and can be used as first line of treatment.
