Suggest Treatment For Back Pain, Tingling In Foot, Heavy And Tight Right Leg

Question: Hi Doctor,

I am 24. I have had problems with back pain since December 14. I have had 2 MRI's done - XXXXXXX & Jun 2015. The symptoms were originally sharp shooting pains in back, heavy and tight right leg, Difficulty in walking, sleepingstanding, sitting.

Now the symptoms are mild to moderate tingling in the foot. Back spams prevent any kind of forward bending. prolonged sitting beyond 40 min and prolonged standing beyond 10 mins is taxing. I am currently paying attention to sitting and all other posture corrections. Along with i am doing XXXXXXX back extension exercises 3-4 times a day and standing backbends very often throughout the day. i have not been advised anything particular on diet or oral supplements. I have had discussions with ortho and neuro specialists and more or less they share the same opinion that its not very serious at this point of time, but requires a lot ot time to strengthen and maintain the spine.
My request to you in particular is the curiousnesss of ayurvedic angle to this treatment and how can i benefit or is there something different i can do to get better faster and resume normal fitnesss activities. I plan to go for graduation studies in september 2015. I am looking to get fit before that. Is that possible by any means ?

Thanks for contacting Ayurveda with your health concern and following guidelines provide better Ayurveda management:

1. Since you have already consulted Neurologist and Orthopedic specialist which are of the view that nothing serious is there thus do let me know about following things:

i. Since there is weakness in legs was any Neurological deficiency ruled out?
ii. Since there is spasm, tingling sensation were SLR, EHL tests carried out by Orthopedic surgeon?
iii. Does MRI shows any nerve root compression, disc buldge, or oedema?
iv. If there is shooting pain in leg was any Pelvic traction, epidural injection, or any surgery performed?

2. Ayurveda management depends upon the type of "Dosha" being involved in the pathology and going by your history it seems to be VAAT dominating in your Spine which is getting aggravated by diet and lifestlye,thus do let me know:

i. Lack of exercise and bad posture?
ii. Eating of excessive dry/cold food stuff?

3. Dietary Regime which you can follow ( in case the asked investigation/examination) comes normal is/are:

I. Take: old brown rice, black gram, horse gram,brinjal, XXXXXXX pomegranate,lemon, grapes,milk,coconut water,meat soup, massage, dry sudation, hard mattress, almonds, raisins, milk cream.

Avoid: newly harvested rice, peas,green gram, jamun, beans, leafy vegetables, honey, grief, late night awakening, remaining empty stomach, excess physical exertion and walking.

PS. If MRI report says that there is no Oedema inside then you can go for massage otherwise not since if oedema is there the said procedure will further aggravate the symptoms.

4. General Instructions:

i. Maintain straight back posture while sitting and standing.
ii. Avoid bending forward and lifting weights.
iii. Use lumbo sacral belt to support the spine and prevent bending.

Do revert back with your clarifications so that Management part can be incorporated in your daily routine.

Hi Doctor,
Thanks for the quick reply.

1. I am attaching the latest MRI report and some film snaps.
2. The neuro doctor tested the foot raising ability personally and based on that ruled out neuro deficit or motor weakness. I have showcased the latest MRI scans only to online neuro and spine specialist via Healthcaremagic. Not personally yet.
3. I am not sure about EHL tests , but SLR tests were conducted in the initial acute stages during Jan-Feb. Now my physiotherapist recommended specefic stretches from March - May, after which the spams and heaviness had eased out significantly. But May onwards the tingling sensation had started, due to which he asked me stop all those stretches and recommended some tuck in of buttock to ease lower back tension. Thus i went for an MRI and will be visiting physiotherapist again to showcase the MRI results. Until then i am on a routine basis sticking to XXXXXXX routine + standing backbends.

4. MRI does indicate a mild compression at l5-s1 level . This is what neuro and ortho specialist told me looking at the attached films and scans. They said nothing major but requires a significant amount of exercises and discipline and strengthening.

5. No Traction / Surgery / injections taken.

6. Prior to onset of back pain. as such there was no dedicated and consistent exercises routine that i followed. But i always used public transport + walk to my job. And thusi believe i wasnt totally inactive by any sense. Also a week prior to onset of back pain in December , I had a team outing, where without mcuh of a warm up , i had played a lot of sports - tennis, golf, cricket on a single day. i believe this caused a tremendous amount of strain which i did not immedietly realise.

7. What is exactly dry/cold food stuff ?? I eat normally . south XXXXXXX veg food. I dont eat fast foods. I rarely eat oustide , probabyl once in 2-3 months. But my office does serve caterers food, which is by definition outside food. But i avoid excessive oily, sweet and fried food whenever i face it.

8. I use lumbar roll whenver i sit to assist my back .

9. I am not sure if my MRI does indicate oedema , since none of the doctors have told anything on this line. Can you please let me know if there is and accordingly suggest the course of treatment along with of course what my physiotherapist suggests??

10. I sleep early daily - around 10.30 - 11 and get up at 6.30 to 7.00 pm.

11. I have not taken any professional massage, I am not sure i can find a good reliable one nearby too.

12. I do a lot of standing backbends .. almost 3-4 times every 45 mins throughout the day. Is this OVERDOING ?? or is this harmless and beneficial ???

Dear
Thanks for reverting back and following is next step of management:

1.     SRL and EHL weakness must be tested in every case of backache [thus check with orthopedic surgeon]
2. Dry/cold food stuff has two meanings:
i. Anything which is not liquid, is refrigerated/packed and aggravates VATA.
ii. Related with taste i.e. sweet/bitter/astringent have COLD energy and as such aggravates VATA.

PS
. Sweet is an exception since its post digestive effect is Sweet as such it doesn't aggravates VATA Dosha.
. Bitter,Pungent, and Astringent foods are less beneficial because they DRY the system, and intensify emotional instability [especially insecurity] on the other hand Sweet,Sour and Salty foods are generally good since they satisfy the system and reduce insecurity.

PS. Large amount of any TASTE should be avoided because VATA is aggravated by excess so check with a qualified Ayurveda Physician regarding your XXXXXXX [Body constitution].

3.     Since there is mild disc bulge with nerve root compression [right side] which could be causing Tingling in legs [get a personal check up with Orthopedic Doctor.]

4.     Since Oedema is not significant in reports you may contact a qualified Ayurveda physician for Panchkarma prcocedure:
i. Kati Basti and Vaitharana basti [oleation],
ii. Avgaha-pinda-nadi sweda, [sudation] and
iii. mridu samshodhan [mild purgation] which gives quick results.

5.      Fry 250 ml ginger juice in 250 ml of til tail, when 250 ml remains [make sure oil doesn't get burn], filter it after cooling and massage.
. Boil 1 spoon dried ginger [सौंठ] in 4 cups of water till 1 cup remains, filter it and add 2 spoon castor oil and drink.

6. Spine Management:
i. Relax the spine completely.
ii. Do not lie face down.
iii. Lie on your back with pillow under the knee.
iv. Lie on the side with a pillow between the knees.
v. Distract your mind from the pain, watch television, read light humorous stories, listen to soothing music.

vi. Take:
4 spoons of XXXXXXX juice
4 spoons of ginger juice
4 spoons of honey and take for 1 month. [empty stomach in morning]
vii. Boil 100 gm black pepper powder in 300 ml til tail--- filter, cool and massage OR mix dried ginger in castor oil and massage.

7. Prescribed Ayurveda formulation which can be taken are:
. Maha Rasnadi Kwatha
. Tryodashang Guggulu
. Mahamash –Mahavishgarbh Taila.
. Dashmool Kadha, XXXXXXX Arishta

8. Your Orthopedican and/or Physiotherapist will be in better position to comment on the exercise schedule that whether overdoing is beneficial [muscle strengthening] or harmful [Aggravating]
